I, Rosa_R. Jumao-as, being the duly elected President of OneSource, OneHeart,

OneDream Consumers Cooperative _of Misamis Oriental Chapter, an association organized with
the sole purpose of mutual help, mutual protection and cooperativism to pursue businesses that will
enhance the financial condition of our members with principal office address at Poblacion Opol
Misamis Oriental Philippines do hereby certify that during the regular General Membership and
Organizational Meetings held on March 23,2019 held at the Poblacion Opol Misamis do hereby
certify with concurrence from all the members present with their names and signatures as
attached certify that the following resolutions were unanimously adopted:

“RESOLVED, that our affiliation and membership with OneSource, OneHeart, OneDream
Association to serve as our national umbrella association is hereby discussed reaffimed and
approved during the said General Membership and Organizational Meetings on March 23,2019

“RESOLVED, that we have fully adopted and ratified the Constitution and By-Laws of
OneSource, OneHeart OneDream Association.

“RESOLVED, that the following are the elected Officers of OneHeart, OneDream
Consumers Cooperative-Misamis Oriental Chapter and to serve as such until revoked or a new
set of officers shall have been elected, to wit:

PRESIDENT- Rosa R.Jumao-as-Zone 8 Clara Poblacion Opol Misamis Oriental

VICE PRES- Elsie R. Abdon –Zone 4 Patag Cagayan De Oro City

SECRETARY- Violeta M. Bactong –Upper Balulang Cagayan De Oro City

TREASURER- Evelyn S. Sabanal –Zone 5,Poblacion Opol Misamis Oriental

AUDITOR- Ernesto C. Abdon – Zone 4 Patag Cagayan De Oro City

“RESOLVED, that to support our affiliation and membership, the following statements
are made:

1. The OneSource OneHeart, OneDream Consumers Cooperative-Misamis

Oriental Chapter is currently in the process of being registered with the CDA
and therefore has its own set of Officers, Board of Directors and members, the
listing of such names are as attached;
2. The organization is not a party respondent in any case pending with regular
courts or administration agencies.
3. The OneSource, OneHeart, OneDream Consumers Cooperative-Misamis
Orienta is willing to cooperate, patronize and support the products, projects
and
 programs of the Association such as the current Grocery Program where the
members will be trained to shop online through the website of the mother
association and promoted at discounted prices up to 50 discount from
grocery products.
4. That our Cooperative will source and produce its owns products for inclusion
in the Grocery Program of the Association.
5. That the Redemption Program benefits will also redound to the members of
the OneSource, OneHeart, OneDream Consumers Cooperative- _________
Chapter.

“RESOLVED, that the following are to serve as members of the Board of Directors and Officers
as elected by the General Assembly and to serve as such until revoked or a new set of officers
shall have been elected.
