AFFIDAVIT
I, GELAINE BAGUI DELGADO, of legal age, Filipino, single and a resident of
Barangay Mabuhay II, Socorro, Oriental Mindoro, after having been duly sworn in
according to law, hereby depose and say:

That sometime in the year 2017, I graduated at Mindoro State College of

Agriculture and Technology (MinSCAT), Calapan City Campus, Masipit Calapan City,
taking up Computer Hardware Servicing;

That after my graduation, I was given a college Diploma by the said school;

That last June, 2019, I rode on the bus going to Manila for the purpose of
applying a job, when I was in the office where I had my interview, I noticed that all my
documents including my College Diploma was lost. That I exerted all my efforts to locate
my College Diploma in all possible places where I believed it could be found, but my
efforts proved futile and in my personal belief the same is now beyond recovery;

That years have elapsed and again I am now applying for another employment, I
was told to submit my college diploma,

That I went to MinSCAT, Calapan City Campus to request for another copy of my
college Diploma, however, I was told by the school registrar that I have to submit to
their office an affidavit of loss showing the loss of my college Diploma and in
compliance thereto, I am submitting this affidavit;

That I executed this affidavit to confirm the truth of all the facts herein stated and
to request the Registrar’s Office of MinSCAT, Calapan City Campus, Masipit Calapan
City to issue in my favor another and/or new college Diploma in lieu of the lost ones and
further, for such other legal intents and purposes this may serve.
