I, Donna Mae Dumaliang, of legal age, single, Filipino, and

residing and postal address at #438 San Diego Street Sampaloc

Manila, after having been duly sworn to in accordance with law in an

answer to the question asked by Atty. Paul Liwanag in his office at

111A, Malumanay Street, Teachers Village, Quezon City, Metro

Manila on August 29, 2019 at 10:30 AM full conscious that I do so

under oath and I may face criminal liability for false testimony or

perjury hereby depose and state:

1) Q. Please state your name and other personal circumstances

for the record.

A. I am Donna Mae Dumaliang, single, Filipino, and residing

at #438 San Diego Street Sampaloc Manila

2) Q: Do you know why you’re here?

A: Yes. I wanted to file a case against Jaime Javier for the

collection of the sum of money she owed me.

3) Q. How do you know Jaime Javier?

A: She was my friend for 18 years and I treated her like a sister

ever since.

4) Q: When did Jaime borrow money from you?

A: On November 07, 2018, Jaime Javier asked me to lend her

Php 500,000 for the hospitalization of her mother, Juanita

Javier.

5) Q: Do you have any proof that indeed Jaime borrowed money

from you on the said date?

 A: Yes. We executed a document in the presence of Patrick

Fernandez as Barangay Kagawad, Katrina Bernabe as

Barangay Secretary and Sandra Lou as Barangay Captain

where we had acknowledged her indebtedness and where she

had promised to pay me the sum of Php 500, 000 on April 07

2019, (Exhibit “A”)

6) Q: Did Jaime pay her indebtedness on April 07, 2019?

A: She failed to pay the said obligation on due date.

7) Q: How did you settle this matter?

A: I approached her personally to demand the payment.

8) Q: Did she answer your demand?

A: She failed to comply with such demand. According to her,

she was experiencing problems regarding her business and at

the same time she’s having problems in taking care of her

mother.

9) Q: What did you do next?

A: she promised me to pay me on June 7, 2019 in full, to which

I have agreed.

10) Q: Did Jaime pay her indebtedness on June 07, 2019?

A: No. She still refused to pay me for the same reasons, i.e.,

the problems regarding her new business and her mother.

11) Q: what did you do next?

A: I approached Atty. Paul Let to formally send letter of demand

payment of their indebtedness. (Exhibit “B”)

 12) Q: Did she answer your demand?

A: She failed to comply with such demand.

13) Q: what did you do next?

A: I tried to settle the matter with her with a Barangay

Conciliation proceeding, but it was futile.

14) Q: Do you have any proof that Jaime indeed refused to

settle his indebtedness?

A: Yes, A Certificate to file Action (Exhibit “C”)
