CONTRACT OF EMPLOYMENT

KNOWN ALL MEN BY THESE PRESENTS:

This EMPLOYMENT CONTRACT made and entered into by and between:

NAME OF THE COMPANY, an entity duly organized and existing


under by virtue of the laws of Republic of the Philippines, with
business and postal address at -
___________________________________ duly represented by
its ________, ___________ hereinafter referred to as the
EMPLOYER.

MR. NAME OF EMPLOYEE, of legal age, Filipino citizen,


Single/Married, and with postal address at
____________________________, herein after referred to as
the EMPLOYEE.

WITNESSETH

1. The EMPLOYEE shall be employed as a / an:

2. The Employment shall be for a period of six (6) months to take effect 20th of July year 2017 and shall expire
on 19th of January year 2018 with equivalent monthly compensation of Fifteen Thousand Pesos (Php
15,000) that only such amount which shall be established by the EMPLOYER to have been duly earned by
the EMPLOYEE during the particular period will be due and payable to the latter;

3. That as an EMPLOYEE, he/she shall acquire the status of:

4. The EMPLOYEE shall assume the position of _________________________.

5. The EMPLOYMENT CONTRACT shall automatically be terminated without the need of notice on the part of
the employee upon the lapse of term herein agreed upon. However, this employment may be renewed at
the discretion of the EMPLOYER before the lapse of the period agreed upon herewith;

6. Either party may pre-terminate this contract by serving a written notice at least thirty (30) days prior to
the intended date of termination. However, the EMPLOYER has an option to shorten the thirty (30) days
notice as deemed necessary;

7. The EMPLOYEE agrees that his work performance shall be properly assessed by EMPLOYER and the
assessment report shall be used as the basis to determine the fitness of the EMPLOYEE for regularization.
However, nothing in the contract shall prevent the parties from renewing or extending this contract;

8. Should the EMPLOYER discover that the EMPLOYEE prior to employment had concealed the fact that
he/she is inflicted with highly contagious disease and/or stated misrepresentations in his/her application,
the EMPLOYER may resort to terminate the services of the said EMPLOYEE without prior notice;

9. The EMPLOYEE shall perform the duties inherent and incidental to the position (as shown under a detailed
job description) and such other duties as may from time to time be assigned by his/her superior;

10. The EMPLOYEE shall be bound by the rules and regulations of the EMPLOYER including those that shall later
on be promulgated;

11. The EMPLOYEE shall not reveal to any person outside the EMPLOYER the trade secret and confidential
information which shall include but not limited to market information, marketing hypothesis and strategic
marketing intent, any technical information, supplier and customer lists and profiles, product rates,
computer systems and business plans of whatever nature that may come to the EMPLOYEE’S knowledge
during his/her employment;
12. The EMPLOYEE shall keep in complete secrecy all confidential information entrusted to him/her shall not
use or attempt to use any such information in any manner, which may injure or cause loss either directly or
indirectly to the EMPLOYER or its business.

13. The EMPLOYEE shall at all times keep safe and secure all the documents and information covered by the
provisions of paragraphs 10,11 and 13 which shall become the property of the EMPLOYER;

14. The EMPLOYEE shall not do anything whatsoever to infringe the copyright, design or patent of all properties
or products of the EMPLOYER, make copies of the properties or products for his or her use for distribution
to other, shall not use any of the properties other than those supplied and authorized for use by the
EMPLOYER and shall not procure any person to do any such infringing acts;

15. The EMPLOYEE shall be liable to pay all damages and/or full indemnity to the EMPLOYER as the case may
be for any liabilities, claims or damages or loss which the EMPLOYER may suffer arising from any
infringements by the EMPLOYEE of any copyright or design:

16. The EMPLOYEE shall be prohibited from (a) assuming any direct or indirect interest or be employed in any
capacity in any other company engaged in business, or (b) exercising his/her profession during his/her
employment unless upon due assignment or authorization from the EMPLOYER;

17. In the event that the EMPLOYEE violates any of the above terms and conditions, especially paragraph 9, 10,
11, 12, and 16 hereof, the EMPLOYEE is bound to pay damages and recognizes the right of the EMPLOYER
to restitution, forfeiture of benefits, termination including the right it file a case for damages to protect its
rights and interest.

18. Should there be any dispute/controversy arising from this contract, the action/suit shall be filed exclusively
with the regular courts of Quezon City, Philippines.
