SURVEY REPORT

INTRODUCTION:

We are M.Sc. Nursing final year student from Government Collage of Nursing, Jagdalpur on the date
24/2/2019 .We had visited Urban PHC Jachacha Bachcha centre (Geedam Road) ,JAGDALPUR . The
PHC is situated near the Jagdalpur and it is under the Baster District.

We had visited PHC with our respected teachers Ms. Shradha Ashawan, Asst. Professor (H.O.D) .Dept.
of Community Health Nursing), and Mrs. Sabiba Daharia (M.Sc. Demonstrator)

DESCRIPTION OF PRIMARY HEALTH CENTRE

INTRODUCTION:

Primary health centre is the first contact point between village community and the
medical office. The PHC were envisaged to provide an integrated curative and prevention health care to
the rural population primary health centre cover a population of 30,000 people

DEFINITION:

“Primary health centre referred to as public care facilities in India. They are part of the
government funded public health system in India and are the most basic unit of this system.”

LOCATION OF PRIMARY HEALTH CENTRE

The PHC is located near by the Jagdalpur , it is away from 4 km from the our college .

FUNCTION OF PRIMARY HEALTH CENTRE

 Maternal and child health services.


 Reproductive child health and family planning.
 Immunization services.
 First aid referral services.
 Laboratory examination like Hb, urine, sugar and albumin.
 IUD insertion.
 To counsel and educate individual family and community.
 To provide family welfare services.
 To collect vital statistic.
 To maintain and records and reports

ACTIVITIES OF PRIMARY HEALTH CENTRE

 Conduct immunization programme


 Anganwadi services.
 School health programme.
 Control of epidemic.
 Antenatal visit and care.
 Screening of woman having menopause.
 Providing health education regarding topics-
 Sanitation.
 Health hygiene.
 Diet
 Proper medication.
 Providing of safe water supply and basic sanitation.
 Nutrition health programme.

house made up of cement and brick .

SUMMARY - In this way we are completed survey of urban area. There are 500 houses in Rajendra
Nagar, some are kchacha houses and some are pakka houses. Mainly Hindi language is used for
conversation.

Most of the families are nuclear family, and there are total population 2519 in which the female
is higher than the male. Education level is also very good. Most of the people is used mobile phone for
conversation .They use motor vehicle for transporting. Most of the houses use electricity and having
proper water supply, occupational level is

low unemployment member is more, and in Rajendra nagar sickle cell anaemia patient is more than 21%.

ROLE OF NURSE:-

I have completed my survey-I got the most of the people is suffering from sickle cell anaemia and
malnutrition.-

1. The nurse should be gave health education during home visit about high iron rich diet(spinach,
grains, soya been, egg, cauliflower, drumsticks, lemons etc.)
2. If the mother is pregnant or lactating so I advice to take the medicines of iron and folic acid tablets
from the PHC.
3. Encourage the propels to take frequently diet and balanced diet.
4. If the children’s is malnourished so aware the community peoples about the programmes-
 Mid day meal programme (To send the Anganwadi center his / her child for education and
nutritional purposes.)
 To awareness about ICDS (integrated child development services) programme of the community
people.
 Nutritional programme for adolscencent girl (Kishori Shakti yojana) aware about diet, menstrual
hygiene, awareness about health, family care, going back to school. All these facilities is included
in this programme.

NATIONAL NUTRITIONAL ANAEMIA PROPHYLAXIS PROGRAMME-

 Weekly iron and folic acid supplementation programme adolscencent.


 IFA tablets target population on weekly basis on fixed day (Monday) for 52 weeks.
 Biannual deworming (February to august).
 Educate the mother about iron rich diet.

The nurse should be educate about hand washing procedure-

 Before eating foods.


 After defecation.
 Before preparing food.
 Wash the vegetables properly before cook etc.

 The community nurse also gives health education about proper disposable of waste material.

The community nurse give health education nurse give health education regarding menstruation and
personal hygiene-

 Proper cleaning of pereneal area with plan water.


 If bleeding is more than should change the sanitary pads time to time between 4-6 hours gaps.
 In case of cloth use, it should be clean properly with dettol, dried in sunlight in order to make it
free from infection.
 Proper disposable of sanitary pads..
 The cloths should be placed in private place.
 The hands should be clean properly.
 Don’t apply soap in the pereneal area.
