Orientation to Basic health unit

It is the first level of health care considered to be necessary to maintain adequate health and
protection from disease. Typically, each Union Council has a BHU which has 2 beds and serves
a catchment population of about 10,000 to 25,000. We visited the BHU of Shehbaznagar situated
at the distance of 12km from Madinah Teaching Hospital. Pervious Prime Minister Shahbaz
Sharif and IqbalNagra inaugurated basic health unit and thirty million expended on its
construction. It is charged with the roles of first level referral for patients referred by LHWs,
primary level organized curative care using approved essential drugs list. The total area of BHU
is 5 acre.

BHU is the first level of care for villagers. The medical officer of basic health unit
Dr.AnilaFayaz provides us the orientation gave all the information about BHU. It is a health
delivery dispensary for public and people had easy access to BHU. There is one vaccination
room, one labour room, one family planning room, and one medical officer room, and one store
room, one ward with two beds, one lady health worker room and one dispensary. Residential
facilities were also available for medical officers and other health workers of basic health unit.
Basic health unit consist of 10 members from medical officers to sweepers. All members
perform their assigned duties. Lady health and community health visitors visit all nearby areas of
community to provide adequate health facilities to mother and child. She also provides
guidelines for family planning to women of reproductive age.

2.2 Indoor and outdoor services:

Basic health unit provide health facilities for 24 hours. It provides indoor and outdoor services to
all community. There was digital system of admissions and medicine entry of all patients. A tab
is used for this digital system. It is a very effective method of keeping records. Basic health unit
provides curative care for pneumonia, pelvic inflammatory disease, scabies, Diarrhea, road
traffic accidents, and acute respiratory infections. They treat the traffic accidents those require
only 2-5 stitches and minor surgeries while more complicated patients referred to RHC or DHQ.

Family planning services and delivery system was also available in basic health unit. Medical
officer said that 2-3 cases normal deliveries perform there once a month. The condition of the
labour room and ward was not adequate or satisfactory for mother and child health. There was
not even a CTG machine in labour room. Sanitary condition was also unsatisfactory and can
cause serious risk of getting infection to mother and child health. Autoclave was also available
but the health workers did not bother to sterilize the instruments. Vaccination facilities also
available in basic health unit. Vaccination of tetanus, inactivated poliovirus, measles, BCG,
pneumococcal, influenza, mumps, and diphtheria were also available there. There was no
management of emergency cases and shortage of modern technological facilities.

3.Orientation to rural Health Center (RHC)

We visit the Rural Health Center to complete the field project of community health nursing
located in “SatianaBunglaw” that is a little City in Tehsil Jaranwala District Faisalabad, Punjab,
Pakistan. It is 27 km from Faisalabad, 25 km from Jaranwala and 23 km from Tandlianwala and
52 km from Madinah Teaching Hospital. It is situated on the Tandlianwala-Faisalabad road.

3.1 Departments:
Departments in RHC are Gynae department, Dental department and Emergency department.
3.1.1 Gynae department:

200-250 patients come on daily basis related to gynae and obstetrics problems on OPD basis.2-3
deliveries of baby done on each day and facility of two deliveries can be done at the same time.
Normal deliveries and episiotomy done and facilities of C-section also available but complicated
cases like breach pregnancy and Pre-Eclampsia not done in RHC but refer to DHQ. Facility of
incubator also available for new born baby and the entire instrument are sterilized to prevent
from infection.

3.1.2 Dental department:

Dental department of RHC facilitates 800 people come with dental problems. Minor surgeries
related to dental problems done.
3.1.3 Emergency department:
Emergency department of RHC deal with emergency cases like patients with diarrhea, dysentery,
enteric fever and also cases of minor to mild bleeding are done. Minor stitching and dressing
done to stop mild bleeding.But in case of severe bleeding patients referred to the DHQ.
