AFFIDAVIT OF DESISTANCE

I, RIZEL A. LACUARIN, Filipino, of legal age, and a resident of Purok 1, Poblacion 3,

Villanueva, Misamis Oriental, upon my oath do hereby depose and state – That:

1. I am the same Rizel Amel Lacuarin who is the private complainant in the case for
Violation of Republic Act No. 9262 (Anti Violation against Women and their Children) filed
before the Regional Trial Court, Branch 22, Cagayan de Oro City against my live-in partner
Corsino M. Maderal.

2. That after conferring with each other, my partner and I have finally come to terms with
each other and have concluded that the incident narrated in my Affidavit-Complaint in
relation to the Anti VAW-C case against Corsino M. Maderal was a simple
misunderstanding between live-in partners.

3. That my partner, Corsino M. Maderal, has made me understand, and addressed my

concerns to my satisfaction and that consequently, I am no longer seeking any civil
and/or monetary claims against him in relation to the VAW-C incident complained of.
That I and the children of Corsino M. Maderal including his immediate and extended
family members have agreed to live harmoniously as one family.

4. Moreover, I further emphasize that I am no longer interested in pursuing any criminal

case against Corsino M. Maderal for Violation of Republic Act No. 9262 (Anti VAW-C) or
for any another criminal case for that matter.

5. I, Rizel A. Lacuarin fully understand the implications of executing this Affidavit of

Desistance consistent with my desire to drop the Anti VAW-C Complaint against Mr.
Corsino M. Maderal, in its entirety, as if nothing has been filed against him.

6. In view of the foregoing, I, Rizel A. Lacuarin, therefore COMPLETELY AND ABSOLUTELY

abandon my Complaint for Violation of the provisions of Republic Act No. 9262 as well
as other causes of action that may possibly arise from the incident against the accused,
Corsino M. Maderal, filed before Branch 22 of the Regional Trial Court of Cagayan de
Oro City.

IN WITNESS WHEREOF, I have hereunto set my hand this May 29, 2019, City of
Cagayan de Oro, Philippines.
