The undersigned petitioner, _________________ unto this Honorable Court, most

respectfully states that:

1. She is a Filipino citizen, single, over twenty one (21) years old. Copies of the
picture of the petitioner with signature at the bottom part are hereto attached
as Annex “A”;

2. She is presently residing at Purok Omar Shariff, Shariff Kabunsuan, RH-3,

Cotabato City, Philippines and residing therein since 2008. She maintains a
regular place of work in the City of Cotabato;

3. The following are the pertinent data in connection with this petition;

4. She has satisfactorily completed the required course in law at Notre Dame
University – College of Law on March 25, 2017;

5. She has passed the 2018 Bar Examinations and admitted to practice law in the
Philippines on June 14, 2019. Copy of the Certificate of Bar Membership
is herewith attached as Annex “B” and made an integral part of this petition;

6. She has no pending Administrative case as evidenced by the certification

issued by the Office of the Bar Confidant which is hereto attached as Annex
“C”;

7. She is a bonafide member in good standing at the Integrated Bar of the

Philippines - Cotabato Chapter. Her Certificate of Good Standing from
two executive officers of IBP Cotabato is herewith attached as Annex “D”;

8. She has paid her IBP Membership due on June 10, 2019 as evidenced by IBP
Official Receipt No. 088383 issued by the Integrated Bar of the Philippines
hereto attached as Annex “E”;

9. She has paid her Professional Tax for the fiscal year 2019 under PTR Receipt
No. 2165811. A copy is hereto attached as Annex “F”;
 10. She possesses all the qualifications and none of the disqualifications of a
Notary Public;

11. She has attached herewith and made integral parts hereof her specimen
signatures as Annex “G”;

12. The undersigned has not been charged nor convicted in the first instance of
any crime involving Moral Turpitude;

13. She has not applied for commission before any other jurisdiction;

14. She undertakes to inform the Executive Judge within ten (10) days in case of
any change in the given address and to submit her monthly report religiously;

15. She now desires to be granted Notarial Commission as Notary Public for and
in the City of Cotabato and Municipalities of Maguindanao.

16. In compliance to the requirements of the Honorable Court for Notarial

Commission, the following attachments are being made:

a. NBI clearance - Annex “H”;

b. Proof of payment of Petition and Summary Hearing’- Annex “I”;
c. Proof of payment of Publication - Annex “J”;

Honorable Court, after due notice, publication and hearing that:
