ABSTRACT
During past decades, trauma has been the major cause of death in persons aged
under 45. As an expression of advance of researches in the field of traumatology,
nowadays there is more interest in the analysis and comprehension of immunologic
processes with repercussion in course and confrontation of organism to injuries,
determining the final prognosis of these patients influencing inexorably in the
appearance of complications, mainly of infectious type, as major causes of the so
called late deaths due to trauma. The characteristic and the main elements
conforming the immune system, the close relation among the different components
of the called psycho-neuro-immune-endocrine integration system and its function in
face of stress, as well as the immune dysfunction after a severe damage are
evidenced to deepen in the study of phenomena characterizing the immune
alterations after a trauma. Thus, authors carried out a revision from the printed
reference indexes, journals and national and international updated texts on this
subject. We conclude that traumata lead to immunosuppression and
hyperactivation, thus the immunomodulation must to includes not only the
restoration of depressed immune responses, but also the regulation of exaggerated
immune responses when it is necessary.

INTRODUCCIÓN
Las lesiones graves constituyen una amenaza intensa para el individuo, pues dan
lugar, entre otros fenómenos, a una gran disfunción de los mecanismos de defensa
del huésped. El organismo puede afrontar una lesión moderada; no obstante,
cuando es grave y agobiante, con necrosis hística extensa, el sistema inmunitario
es activado sistémicamente y puede volverse autodestructor, con el consiguiente
incremento de la morbilidad y la mortalidad en los pacientes lesionados.2
Debido a su gran importancia y al interés que despierta cada vez más en los
especialistas, se decide realizar una revisión actualizada, con el objetivo
fundamental de profundizar en los fenómenos que caracterizan la respuesta
inmunitaria en el paciente lesionado, quien sometido a estrés por múltiples causas
(físicas, emocionales y quirúrgicas) presenta, desde etapas muy precoces,
alteraciones inmunes importantes que determinan el pronóstico final. Para ello se
realizó una búsqueda automatizada exhaustiva y el estudio manual de índices
referativos impresos, y de revistas nacionales y foráneas, de actualidad, que tratan
el tema en cuestión.

EL SISTEMA INMUNE
Los linfocitos Th1 producen IL-1, interferón gamma (IFNγ) y factor de necrosis
tumoral alfa (TNFα) que activan los macrófagos y las células NK, mientras que los
clones Th2 secretan IL-4, IL-5, IL-6, IL-10 e IL-13 que modulan la producción de
las distintas clases y subclases de inmunoglobulinas (lg) por los linfocitos B. En
determinadas situaciones existe una relación antagónica entre ambos clones. Las

citocinas que generan las Th1 inhiben el desarrollo de las células Th2, mientras que
las IL de las Th2, en su mayoría antiinflamatorias, anulan las acciones
proinflamatorias de las Th1. Esto es importante, pues evita respuestas desviadas,
erróneas e, incluso, dañinas para el paciente.3,9
La célebre frase hipocrática "mente sana en cuerpo sano" demuestra que desde
tiempos antiguos se ha observado la asociación entre situaciones de estrés físico y
psicológico con la génesis de enfermedades, sobre todo infecciosas.9

Las lesiones graves dan lugar a grandes desarreglos en los mecanismos de defensa
del huésped.2 Estos procesos están determinados por una serie de factores
causales: unos intrínsecos (predisposición genética y estado fisiológico) y otros
extrínsecos (tipo o grado del insulto e intervenciones quirúrgicas).14

sentido más útil según las nuevas necesidades y donde el eje psiconeuroendocrino
tiene un papel protagónico.32
Sin duda, las complicaciones en los pacientes lesionados, sobre todo las de causa
infecciosa, constituyen un contribuyente principal de las tasas de morbilidad y
mortalidad tardías después de la lesión (second hit o segunda fase de la DOM).21
Por lo tanto, esto trae consigo que el apoyo de la resistencia del huésped se vuelva
muy significativo.
