Acute phase (0-2 weeks)


Goals
o Reduce pain
o Initiate passive shoulder range of movement
o Maintain elbow/wrist range of movement
Rehabilitation
o Shoulder passive range of movement exercises within pain/stretch limits
o Elbow/wrist range of movement exercises
o Scapular setting exercises
Criteria for progression
o Reduction and control of pain

Intermediate phase (2-6 weeks)


Goals
o Reduce pain
o Wean sling by 6 weeks
o Restore shoulder range of movement
o Scapular control through range
Rehabilitation
o Start to wean sling at 4 weeks post op
o Gentle isometric exercises in neutral as pain allows
o Scapular control exercises
o Begin active assisted exercises & progress to active exercises when sling
weaned
o Hydrotherapy permitted
Criteria for progression
o Pain controlled
o Adequate shoulder range to start rotator cuff strengthening

Late phase (>6 weeks)
Goals
o Functional shoulder range
o Strengthen cuff
o Scapular control
Rehabilitation
o Progress to full active exercises in all ranges
o Rotator cuff strengthening
o Progress cuff and scapula recruitment and control through range
o Closed chain exercises
o Capsular stretching
o Proprioceptive training

Guidelines for return to functional activities


Driving Lifting Swimming Golf
6 weeks Light lifting at 6 weeks. Avoid 6-8 weeks 3-6 months
heavy lifting until fracture
healed.
