Madam:

This is to officially notify you that I am planning to voluntarily forego my employment with the company
in compliance to Art. 285 of the Philippine Labor Code which requires at least 30-day notice upon
resignation. I am leaving due to personal and other reasons.

My tentative end date would be on March 10,20017.

I assure you that I will continue to perform my duties and responsibilities within this period.

I would like to thank you and the management in general for the opportunity and the privilege of
working in this company. My learnings and experiences here will surely come handy in my other
endeavors.

On my end date, I will diligently process my clearance and turn over all company property/ies and settle
any accountability/ies, if there is any.

I will also cooperate on anything that would allow for a smooth transition of my duties as an agent.
