STAGE 3A APPROVAL BY THE PROJECT SAFETY COORDINATION COMMITTEE. CHAIRMAN

I have: (1) evaluated the hazards and the risks associated with the work
(2) coordinated the work activities on the project at the Project Safety Coordination Committee Meeting to ensure that no incompatible work will be carried out by all parties working on the
project.
(3) counter checked that all certificates issued earlier for incompatible operations have been withdrawn
(4) proper assessment of the hot-work area and its surroundings have been made so that the hot-work can be carried out safely.
