This contract is between the students and the counselor outlining specific conditions and terms which are expected to the student in
order to meet academic progress.

Low Grade point average

Absenteeism/Tardiness

Misconduct

CONTRACT TERMS AND CONDITIONS

Required to achieve passing grades

If needed, meet with the adviser, to develop a learning/ tutoring plan, submit the copy of a plan

to the counselor and comply with the conditions set forth the plan

Lessen absenteeism /tardiness

Observe proper conduct otherwise may refer for disciplinary action

Attend the remediation class

Good feedback from academic performance monitoring from as followed up by the counselor

I understand the terms and conditions of this contract and acknowledge that failure to meet any of them may result immediate action from the
counselor.
