MOTION TO ADMIT SUPPLEMENTAL ANNEXES TO PETITION

Petitioner, thru counsel, unto this Honorable Court, respectfully

state that:

1. On August 27, 2019, a clarificatory hearing was held with regards

to the condition, “Dependent Personality Disorder”,
Respondent was diagnosed to be suffering;

2. In support for the said diagnosis of the Respondent, Petitioner is

submitting as Supplemental Annexes to the Petition the
Psychological Assessment Report prepared by Mariflor J.
Gatchalian, Rpsy, CSaP as Annex “H” and that said Diagnosis
found on the same report on page 11 as Annex “H-1”;

WHEREFORE, premises considered, it is most respectfully prayed

of this Honorable Court that the Supplemental Annexes herein
submitted be admitted as part of the original Petition.

Other reliefs and remedies available under the premises are

likewise prayed for.
