CONTRACT OF LEASE

(PARKING SLOT)

It is hereby mutual agreed upon by and between:

_____________________________________, a non-stock, non-

profit corporation, duly registered with the Securities and Exchange
Commission, with postal address at ___________________________ and
hereinafter referred to as the “LESSOR”;

-and-

____________________________________ represented by
Mr./Ms. ________________ of legal age, and with postal address at
_______________________________, hereinafter referred to as
“LESSEE”.

-WITNESSETH-

1. That the LESSOR is the owner of the following Parking Slots 5F-39, 6F-20, 7F-01,
7F-22, 7F-23, 7F-33, at _____________________________________________;

2. That the LESSOR has offered, and the LESSEE has agreed, to lease the above
mentioned parking slot at THREE THOUSAND ONLY (Php 3,000.00) per month,
which shall be billed together with the monthly Statement of Account.

3. That the period of lease shall be for One (1) year beginning on October 17, 2018
and will expire on October 16, 2019. Furthermore, the LESSEE shall notify the
LESSOR, in writing, at least one (1) month before the end of the Lease Period,
whether or not the former wishes to renew this Lease Contract.

4. That the LESSEE may pre-terminate the term of this Contract of Lease by notifying
the LESSOR, in writing, at least Thirty (30) days prior to the LESSEE’s intended date
of termination, provided, that the LESSEE shall pay the LESSOR a penalty
equivalent to One (1) month rental, which shall be paid prior to the termination date;

5. That the LESSEE shall use the Parking Slot in accordance with the Master Deed
and Declaration of Restrictions and the House Rules and Regulations of the
___________________________
