OVERVIEW OF HEMATOCRIT VALUE IN TUBERCULOSIS (TB) PATIENTS


THAT GIVES TREATMENT OF ANTI TUBERCULOSIS MEDICINE (OAT) AT
THE BUNGSU HEALTH CENTER IN BANDUNG

❖ Introduction
Tuberculosis is a direct infectious disease caused by Microbacterium Tuberculosis.
Transmission occurs when TB patients cough or sneeze, germs spread in the air in the
form of droplet nuclei. OAT treatment is performed on TB patients aiming to cure
patients, prevent death, prevent recurrence, break the chain of transmission and prevent
bacterial resistance to OAT. The use of OAT in TB patients based on the long
consumption of OAT can cause a decrease in hematocrit values caused by side effects
of the drug, namely the occurrence of anemia and thrombocytopenia. This study aims
to describe the hematocrit values in TB patients who received OAT treatment at the
Bungsu Health Center in Bandung.

❖ Method
• Research Method
The research method used in this study is descriptive method, which is to
obtain an overview of hematocrit results in TB patients receiving OAT treatment at
the Bungsu Health Center Bandung.
• Sample and Participants
1. Participants
The participants in this study were all TB patients who came to
visit and do treatment at the Bungsu Health Center until April 2019 as many as
30 people.
2. Sample
The samples taken in this study were TB patients who treated the
OAT package at the end of the second month as many as 24 people and at the end
of the fourth month as many as 6 people and carried out hematological
examinations. The sampling technique in this study used a purposive sampling
technique, which took all patients who were on OAT therapy and carried out
treatment at the end of the second month and the end of the fourth month and
carried out hematological examinations to be used as research samples .
3. Research Instrument
In this study using instruments checklist sheets, labels, and
stationery as well as tools and materials.
4. How to Collect Data
A. Patient Preparation
Patients who are temporarily OAT therapy
B. Preparation of Tools and Materials
Handscun, Masks, EDTA Tubes, Spoit, Tourniquet, Sysmex Hematology
Analyzer, 70% Alcohol Cotton, and Plaster.
C. Sample Preparation
1) Fill in the patient's identity
2) Blood Vein Collection
a. Preparation of tools and materials
b. Do the patient's approach calmly and kindly, try to make the patient
as comfortable as possible
c. Identify patients correctly in accordance with the data in the request
sheet.
d. Verify the patient's condition, such as fasting or taking medication.
Note if the patient is taking certain medications, not fasting and so on.
e. Ask the patient to straighten his arm, choose the arm that does a lot
of activity.
f. Ask the patient to clench his fist.
g. Put a damming rope (tourniquet) about 10 cm above the elbow fold.
h. Select the part of the cubital or chepalic median vein. Perform
palpation to ensure the position of the vein.
i. Clean the skin on the part to be taken with 70% alcohol cotton and let
it dry.
j. Pierce the vein with the pinhole position facing up. If the needle has
entered the vein, blood will appear in the syringe.
k. After the blood volume is considered sufficient, ask the patient to
open his fist. The volume of blood taken is approximately 3 times the
amount of serum or plasma needed for examination.
l. Place dry cotton in place of injection and then remove / pull needle.
Ask the patient to press the cotton.
m. Put the blood into the tube provided.
5. How to Analyze Data
A. Automatic checking using the Sysmex Hematolgi Analyzer with EDTA
blood sample.
B. Work Procedure
1) Insert the sample into the EDTA tube
2) Make sure the device is in the ready state press the button (WB) to
change the Whole Blood (WB) mode, then press the button (sample
id). Enter the sample number and press the enter key.
3) Homogenizing the sample blood to be examined properly. Open the
lid and place it under the aspirator probe. Make sure the tip of the
probe touches the bottom of the bottle of blood sample so it doesn't
suck up.
4) Press the star switch to start the process.
5) After two beeps are heard running on the screen and the rise up
down, the sample tube can be taken by lowering the blood sample
tube from the bottom of the probe.
6) Results will be displayed on the screen automatically printed on the
printer.
Results come out in the form of print out Normal if:
Women : 37% - 43%
Men : 40% - 48%
