4) Nursing Course Completed certificate true
copy of all nursing staff
5) Maharashtra Nursing Council (MNC)
registration Certificate true Copy of all nursing

6) Previous copy of hospital registration (for
renewal)
7) Property tax receipt current year/latest true
copy
8) Water tax receipt current year/latest true
copy
9) Declairation on 100/- rs. stamp paper (For
new Hospital registration)
10) Receipt of Charges (Incineration & Nursing
home reg. fees) xerox copy
11) If hospital owner is Homeopathic practioner
then submit Declairation on 100/- rs. stamp
paper, mentioned that he will practice only
homeopathic. also submit undertaking from
alleopathic docter that he is ready to give
service for 24 hour.

number of Beds
i.e. :- 0 Beds to 5 Beds - No
 Charges

6 Beds to 25 Beds - Rs. 1,250/- For

one year
26 Beds to 50 Beds - Rs. 2,500/- For
one year
51 Beds to 200 Beds - Rs. 5,000/- For
one year
201 Beds to 500 Beds - Rs. 10,000/-
For one year
More than 500 Beds - Rs.15,000/- For
One Year
