We are terminating your work for the reason that ____________________________ .

By reason of your violation of our COMPANY POLICY, we have sent you a Notice to Explain dated
_____. To this date, we have not yet received any response.

Your act of ___________ is a grave violation of the Company’s Policy and such violation is meted
with the penalty of termination. For more or additional information regarding the rules/procedure
for terminating employee, you can refer to the COMPANY POLICY.
If there is no Company Policy refer to the Labor Code of the Philippines.
Please be reminded to return all documents and any or all of the issued equipment to you by the
company and turn over the same to your immediate superior.
Likewise, please be do not forget to process your clearance with the HR Department before parting
with the company.
Similarly, we would like to remind you that despite the termination of your work with the company,
you have to settle all your obligation with the company whether monetary or otherwise.
In any event that you failed to pay or settle your remaining liabilities with the company, we will be
constrained to deduct the same from your last pay and/or resort to legal means including but not
limited to filing a civil or criminal case against you.
