ACTIVITY: 5th Alumni Homecoming

Description: School Program on Parents’ Awareness

Objectives: The following were the specific objectives of the conduct of the activity:

a. To disseminate to the parents or stakeholders the Deped Order


No. 03,series of 2018, Basic Enrollment Policy
(Rationale, Statement and Scope of policy).
b. To follow the Enrolment Procedures on Early Registration.
c. To give the requirements or documents during the enrolment
period and the requirements for transferees and special cases.
d. To know the parents the date of first attendance and temporary
enrolment.
e. To identify the transferred in and out, moving in and out of the
learners.
f. To let them know the system wherein the pupils to be enrolled.
g. To accomplish the necessary documents upon enrollment and
release of the cards from kinder to Grade 6 learners.
h. To know the voluntary contributions and homeroom contributions.

RESULTS: The identified objectives and targets for the activity were met.
All objectives were discussed well by the respective discussant and all the
parents listen attentively. The parents identify the necessary documents to
be submitted upon enrollment and they must know the status of their
children in terms of enrolment.

Methodology

The parents known already the necessary documents to be submitted to


the respective adviser on or before August 31st. Failure to submit the said
requirements the status of their children serve as temporary enrolled. The
parents also know their obligations and responsibilities to their children
while schooling. Their voluntary contributions must be paid within the
school year. They know already the Deped Order, No.03, series 2018 and
the guidelines upon enrolment.

Management of the Activity

The orientation were successfully managed by the Anislag Elementary


School Teachers, headed by Mr. Norman T. Cabugawan, Principal II.

Logistics Arrangements

Teachers of Anislag Elementary School headed by our principal Mr.


Norman T. Cabugawan with the concerned authorities helped for the
success of this activity.

ACTIVITY: Second HRPTA Meeting

Venue: Grade 3-1 Classroom

Participants: Parents

Description: Parents awareness for the updates of the school activities

Objectives: The following were the specific objectives of the conduct of the activity:

i. Follow up the attendance, behavior, assignment, reading,


writing and school activities of the pupils.
j. Election of HRPTA Officers SY: 2018 - 2019
k. Disseminate the voluntary contributions and classroom
contributions.
l. Identify the Homeroom Project (Ceiling Fan)
m. Release of books.
n. Remind the documents to be submitted( live birth, PSA)
o. Other matters

RESULTS: The identified objectives and targets for the activity were met.

The following were the outputs based on the objectives of this


meeting.
a. The parents agreed to follow-up their pupils every time.
b. Elected the classroom officers
c. The parents agreed in terms of contributions.
d. The parents agreed their homeroom project (Ceiling Fan)
e. They agreed to submit the documents before August 30. 2018

Participants’
Feedback Only 11 Grade ONE parents attend the HRPTA Meeting , all this
parents agreed about the agenda presented in the meeting.

Methodology

The meeting started with a prayer, and I introduce myself to the parents.
Followed by the presentation of the agenda to be tackled in the first
HRPTA Meeting, so far the agenda were disseminated to the parents and
the meeting was successfully done and one of my parent donate a Stand
fan.

Management of the Activity

The meeting was managed by the adviser: SHALLY G. SAGADAL

Logistics Arrangements

All Grade 1 parents concerned helped for the successful conduct of the
meeting.
