ACTIVITY: FOURTH PORTFOLIO DAY

Description: Porfolio Day


Objectives: The following were the specific objectives of the conduct of the
activity:

a. To conduct classroom meeting


b. To present the test Result, Least Learned Skills and High
Performing Learners.
c. To Exhibit/ gallery of pupil’s outcome and performance.
d. To sign the cards of the pupils.

Preliminaries The pupils make their projects for the 4th Quarter served as their
output/ displays during the Portfolio Day.
All pupils must take the 4th quarter Examination in order to release
the report card to their parents.
Conduct meeting for the preparation of the graduation and closing
exercises.

Design and There were 63 parents attended. There were 103 tentatively pupils

RESULTS:

Output/s Pillow case, apron, slipper/ toy made from indigenous materials,
compilation of their written outputs, arts/ animals made of clay.

Innovations/ Conduct a short homeroom PTA meeting to the parents in order to


initiatives discussed some issues and concerned regarding the status of their
child/ children for their respective classroom adviser.

Participants’ Active participation of the parent were notified in this particular


Feedback activity. Parents have a time to follow up their child/ children to their
class adviser about the status of their learning situations.

Issues and Some parents were not able to come due to the synchronized

ACTIVITY: Second HRPTA Meeting

Description: Parents awareness for the updates of the school activities

Objectives: The following were the specific objectives of the conduct of the activity:

a. Follow up the attendance, behavior, assignment, reading,


writing and school activities of the pupils.
b. Election of HRPTA Officers SY: 2018 - 2019
c. Disseminate the voluntary contributions and classroom
contributions.
d. Identify the Homeroom Project (Ceiling Fan)
e. Release of books.
f. Remind the documents to be submitted( live birth, PSA)
g. Other matters

RESULTS: The identified objectives and targets for the activity were met.

The following were the outputs based on the objectives of this


meeting.
a. The parents agreed to follow-up their pupils every time.
b. Elected the classroom officers
c. The parents agreed in terms of contributions.
d. The parents agreed their homeroom project (Ceiling Fan)
e. They agreed to submit the documents before August 30. 2018

Participants’
Feedback Only 11 Grade ONE parents attend the HRPTA Meeting , all this
parents agreed about the agenda presented in the meeting.

Methodology

The meeting started with a prayer, and I introduce myself to the parents.
Followed by the presentation of the agenda to be tackled in the first
HRPTA Meeting, so far the agenda were disseminated to the parents and
the meeting was successfully done and one of my parent donate a Stand
fan.

Management of the Activity

The meeting was managed by the adviser: SHALLY G. SAGADAL

Logistics Arrangements

All Grade 1 parents concerned helped for the successful conduct of the
meeting.
