AFFIDAVIT OF COMPLAINT

I, MARY ANN D. ZAMORA, of legal age , Filipino Citizen, Married and a resident of
32 Tinagan St., Brgy., San Jose, Quezon City, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and state:

1. That I am a the mother of LANZ JEDRIC Z. NARISMA, Filipino Citizen born on

May 29, 2006.

2. That the undersigned, accuses ERIC S. NARISMA, the Father of Lanz Jedric Z.
Narisma of being severely neglectful of his financial obligation to his son LANZ
JEDRIC Z. NARISMA.

3. That on or about March 2019, ERIC S. NARISMA, sent Ten Thousand Pesos (Php
10,000.00) to his son as his monthly financial support but on April and May 2019,
none was given to his son. After being confronted by the undersigned, he replied that
there was no need because it was school vacation then.

4. That on June 2019, ERIC S. NARISMA, sent Ten Thousand Pesos (Php 10,000.00)
to his son as his monthly financial support, but none was given last July 2019. The
father sent again the same, last August 2019 then none again was given last
September and October 2019.

5. That the undersigned, confronted ERIC S. NARISMA, the Father of Lanz Jedric Z.
Narisma on or about August 2019 of being negligent and inconsistent when it comes
to his financial obligation. The latter responded that, I have no right to question him
regarding the matter because according to him, He and his son was the one who had
an agreement.

6. That the undersigned is demanding that ERIC S. NARISMA, the Father, should
fulfill his Financial obligation as agreed upon to his son, LANZ JEDRIC Z.
NARISMA.

7. I am executing this affidavit to attest to the truthfulness of the foregoing.
