EFPS. For the reason that the past employee who handle the past transactions of our business
to your office have AWOL and can’t provide us the username and password of our Efps.

We would like to have a Re-Enrollment for the Efps. To gained access of our previous
transactions and also to address our nearly dated tax obligations.

I would like to authorized John Dominic R. Aldaba as new authorized user person to
access the Efps. We will attached his ID in this letter.
