I, MST. SUBHAT TAHRIA D/o. AHSAN SHAH, Muslim, adult, holding CNIC #
_____________________, Resident of House No. C-4, Rizvia Society, Nazimabad,
Karachi, do declare solemnly affirm as under:

That I am the deponent of this Khula Nama

That I declare through this Khula Nama that my Nikka Ceremony with Mr. Asfar Akram
S/o. Muhammad Akram in accordance to Islamic Shariyat Law as on 6th January 2015 at
Karachi in presence of witnesses.
That after Nikka with Mr. Asfar Akram; I felt that we could not able to make mutually
understanding so that I consult with my elder in family member and decide to request for
Khula Nama from Mr. Asfar Akaram; so that through briefly clarification I humbly
request to Mr. Asfar Akram to accept my request for Khula Nama.
That in view of aforesaid facts; I also declare that I forget the Haq Mehr amount to Mr.
Asfar Akram in favor of acceptance of this Khula Nama.
That above mentioned statement is stated in presence of two witnesses.

Whatsoever stated above statement is true and correct to the best of my knowledge and
