2019 Benefit Summary


Tallahassee Memorial HealthCare (TMH) is pleased to provide a comprehensive benefit program to our TMH
colleagues and their families. This summary is intended as a brief description of the benefits provided by TMH
colleague benefit program. Each benefit is additionally described in a formal Plan Document, which specifies the
guidelines for coverage, eligibility and the benefits provided to you.
Cafeteria Benefits Plan: The Medical, Dental, Vision and Flexible Spending Accounts (FSA) plans are all part of the
Cafeteria Benefits Plan. The Cafeteria Plan is a benefits program designed to take advantage of Section 125 of the
Internal Revenue Code. A Cafeteria Plan allows colleagues to pay certain qualified expenses on a pre-tax basis,
thereby reducing total taxable income and increasing the spendable/take-home income. Unless otherwise noted
benefits are offered to colleagues who work 60 hours or more per pay period in a benefit designated position.
Colleagues have a choice between three medical plan coverage options: Point of Service (POS), Preferred Provider
Organization (PPO) and Health Maintenance Organizations (HMO). The bi-weekly colleague rates are below:

Colleagues have a choice between three levels of PPO dental coverage options: Standard, Premium and Premium
Plus. The bi-weekly colleague rates are below:

Colleagues have the choice to enroll in our comprehensive vision care plan. The bi-weekly colleague rates are below:

Defined Contribution Retirement Plan [401(a)/403(b)]: TMH contributes to a 401(a) retirement plan on behalf of all
eligible Colleagues (currently 4%). In addition, colleagues may save for their own retirement through payroll
deductions on a pre-tax basis through a 403(b) with TMH matching a portion of the savings dollar for dollar (currently
up to 2%). Our plans are participant-directed with a wide range of recognized mutual fund opportunities.
Flexible Spending Accounts (FSA): A Flexible Spending Account (FSA) is a benefit established under Section 125 of the
Internal Revenue Code. This plan allows you to contribute to a Healthcare Account and/or Dependent Day Care
Account on a pre-tax basis.
Term Life Insurance: Colleagues with $25,000 of group basic term life insurance and accidental death and
dismemberment (AD&D) at no cost to the colleague. Colleagues may purchase additional coverage for themselves in
increments of 1,2,3,4 or 5 times their base annual income up to a maximum policy value of $750,000. Upon hire only,
benefit eligible colleagues can elect up to 3 times their annual salary (maximum of $350,000) at guaranteed issue.
Rates are based on age and base salary. Coverage for a spouse and dependents is also available.
Short Term Disability Income Plan: Colleagues may enroll in our short term disability plan. The maximum benefit can
be up to 60% of pre-disability base pay up to a maximum of $1,250 per week. Benefits may be payable after a 14-day
waiting period for up to 26 weeks. Upon hire only, colleagues can elect disability at guaranteed issue.
Long Term Disability Income Plan: Colleagues may enroll in our long term disability plan. The maximum benefit can
be up to 60% of pre-disability base pay up to a maximum benefit of $5,000 per month. Benefits may be payable after
180-day waiting period. Upon hire only, colleagues can elect disability at guaranteed issue.

2019 Benefit Summary
Whole Life Insurance: Colleagues may purchase whole life insurance which pays a death benefit to your beneficiaries
but it can also build cash value you can use while you are living. Later in life, you can use this cash value to buy a
smaller, “paid up” policy with no more premiums due. This benefit offers an affordable, guaranteed level premium
that won’t increase with age. Coverage is available during the TMH annual open enrollment period.

Critical Illness: Critical illness insurance can help protect your finances from the expense of a serious health problem,
such as a stroke or heart attack. This plan pays a lump sum benefit directly to you - not to a doctor or health care
provider – at the first diagnosis of a covered condition. Coverage is available during the TMH annual open enrollment
period.
Personal Leave Time (PLT): TMH provides eligible colleagues the benefit of a specific amount of accrued paid time off
from work once a colleague has completed the TMH “90-day introductory training period”. For specific guidelines
about this benefit, please refer to policy and procedure No. 45-604.
All eligible regular full-time and part-time colleagues will accrue PLT as follows:

Long Term Sick Time (LTS): Tallahassee Memorial HealthCare, Inc. (TMH) understands that colleagues may be unable
to report as scheduled to work due to their own illness or the illness of a spouse or child, or due to the illness of a
parent. TMH provides Long Term Sick (LTS) payment, as accrued and available to provide compensation to the
colleague for these absences. For specific guidelines about this benefit, please refer to policy and procedure No. 45-
605.
LTS begins to accrue each pay period after regularly employed full-time and part-time eligible colleagues complete six
(6) months of continuous service with TMH. LTS will accrue according to the table below:

Premier Health and Fitness Club: Premier Health & Fitness Center (PHFC) is a 55,000 square-foot facility with the

Childcare Center: TMH owned and operated on-site facility providing developmental learning programs including
Gold Seal VPK. Fees and rates are dependent upon the age of the child and the number of days in attendance.
Tuition Reimbursement: Tallahassee Memorial HealthCare recognizes the mutually beneficial value of an educated
work force. For specific guidelines about this benefit, please refer to policy and procedure please refer to policy and
procedure No. 45-502.
Employee Assistance Programs: Employee Assistance Program (EAP) is a confidential service offered to colleagues
and their families. EAP services are provided at no cost to you. To ask questions or schedule an appointment please

Colleague Discounts: TMH colleagues and family members may receive up to a 50% discount on the self-pay portion
of covered TMH inpatient and outpatient services after receipt of all insurance payments. Exclusions and restrictions
apply.
