Today,______________, ______________________,2018, Employment Agreement has been made and


mutually agreed to by the undersigned:

PT. Kurnia Ciptamoda Gemilang, a company organized and existing under the laws of the republic of
Indonesia, doing business as Retail fashion, represented in this matter by Yuli Santi. as a Human
Resource & General Affair Head Division, whose capacity will be as Company ; and

Mr/Mrs. ___________________________, an Individual residing at _______________________, whose


capacity will be as The Employee

The Company and the Employee have agreed to be bound in the fixed term employement agreement as
per the following provisions :

Article 1
Duty And Placement

1. We are pleased to accepted you to be employed within PT. Kurnia Ciptamoda Gemilang with the
position as ___________________________
2. The employment period is effect from _____________________ till
______________________________
3. Employee agrees to be employed as per the title stated in Article 1 verse 1 and will carry out the
duty as well as possible and in full responsibility.
4. Employee states that he/she is willing to be placed / transferred to other duties and or other
location as required by Company.

Article 2
Working Days, Working Hours And Holidays

1. Work days are 5 (Five) days within 1(one) week starting from Monday to Friday.
2. The working hours of The Employee is 8 (eight) hour per day, the break time starts from
12:00 WIB (West Indonesian Time) until 13:00 WIB (West Indonesian Time), except for Friday, the
break time starts from 11:30 WIB(West Indonesian Time) until 13:00 WIB (West Indonesian Time).
These working hours are subject to change or adjusted to the condition need.
3. Weekly holidays are Saturday and Sunday & Public holidays are determined by the Government of
Indonesia.
Article 3
SALARY

1. Your commencing salary will be :

a. Basic salary : Rp.___________________________/ Month


b. Housing fixed allowance : Rp.___________________________/ Month
c. Functional allowance : Rp. __________________________/ Month
2. Salary is paid on the 30th or 31th of the following month, and if the date falls on holiday date,
payment is to be paid 1 (one) day earlier.
3. Salary adjustment will be made in yearly review on ________________ 20_____ and adjusted
based on performance appraisal.

Article 4
PROBATION

Your employment will be subject to a probationary period of three (3) months during which time either
the Company or yourself may terminate employment on one (1) day’s written notice or payment of one
(1) day’s salary in lieu of notice without any reason being assigned.

Article 5
INCOME TAX (PPH 21)

The company will be responsible for the payment of your income tax on your salary in Indonesia.

Article 6
HARI RAYA ALLOWANCE (THR)

1. The Company will provide Hari Raya Allowance (THR) to THE SECOND PARTY having worked
continuously for 3 (three) months or more.
2. For Employee, who has worked for 3(three) months or more but less than 1(one) year, Hari
Raya Allowance (THR) will be paid proportionally.
3. For Employee, who has worked for 1(one) year or more will be provided THR as much as 1(one)
month fixed salary.
Article 7
OTHER BENEFITS

Upon confirmation, you are also entitled for :


1. Medical insurance will be provided by the company after you have passed the probation period
( 3 Months) and adjusted to the grade of position.
2. Medical allowances applied for Inpatient and Outpatient.
3. The Employee is entitled to 12 (twelve) days annual leave or more adjusted to the grade of
position after completing 3 months probation period.
4. The scheduling of employee’s annual leave must be approved by direct superior.

Article 8
MATERNITY LEAVE

Maternity Leave in full payment of salary will be entitled for female employee who has joined with The
Company for the period of service at least two (2) years continously after signing this agreement .

Article 9
SECRECY AND OTHER CONFIDENTIAL MATTERS

You will not during your employment or after the termination of such employment, utilize or divulge to
any person or persons, any secret or confidential knowledge or information which you may have
acquired as a result of your employment by the Company.

Your shall immediately, upon termination of employment, return and deliver to the Company all
properties belonging to the Company or related to its business including, in particular, materials,
memoranda, notes, photographs, records, files, reports and other documents.

Article 10
CONFIRMATION AND NOTICE PERIOD

At the end of the probationary period, your service will be confirmed in writing. Termination of your
service thereafter will require one (1) month’s written notice.

As………………………., you will be given access to sensitive information and data of the Company.
Consequently, it is necessary to highlight to you the following clauses, non performance of which will
constitute a breach of trust and will result in either disciplinary action or termination of service.
Article 11
CONFLICT OF INTEREST

You shall not during your employment with the Company, embark, engage or interact yourself for
reward or gratuitously in any activity which would interfere with the performance of your duties with
the Company or which would conflict of yours interest with the business of the Company.

Article 12
PUBLICATIONS, INTERVIEWS, BROADCASTS, ETC

You may not, without having obtained the prior written consent of the Shareholders, publish or cause to
be published any article, book, photograph or letter; or give any interview or broadcast or deliver any
lecture of speech on any matter which concern your duties.

Article 13
DISPUTE

The Employee and company agree to use their best efforts to resolve all and differences arising out of
this agreement or the Employement relationship is an amicable and conciliatory manner. In the event
any dispute cannot be resolved, tha matter will be referred to the relevant government authorizes in
Indonesia

Article 14
RULES & REGULATIONS

1. The Employee agrees to strictly adhere to all rules and regulations as established and amended
from time to time by the company. The Employee shall at all times maintain the highest
standards of honestly, integrity, loyalty, courtesy and cooperation and shall at all times uphold
the good name of the company.
2. The Employee shall at all times use the company property in an authorized and shall be subject to
sanctions, including dismissal, in the event of intentional, malicious, or reckless abuse, damage or
misappropriation of the company’s property.

Article 15
CLOSING

In witness whereof, this Employment Agreement is made in duplicate 2(two) adequately sealed and
becomes an integral part of the Statement Letter made and approved by The Employee, and after the
parties have read, understood and signed consciously in physical and mental health without any
coercion, then each party keeps 1(one) set of the original copies.
