AFFIDAVIT WITH SPECIAL POWER OF ATTORNEY

I, MEGUELITA ABOYABOR AMBOYON, Filipino, of legal age, single, a resident of

Poblacion I, Tagbilaran City, Bohol, Philippines, after being sworn to in accordance with law do
hereby depose and say;

1. That I am the biological mother of ZACHARY EZEKIEL AMBOYON MANGUIOB, who was
born on September 6, 2019 in Tagbilaran City, Bohol;

2. That the biological father of the said child is JUNELMAR CHRIS ELEMINO MANGUIOB, as
evidenced by the hereto attached Admission/Acknowledgment of Paternity, Consent to
Use Surname, executed before the Manila Economic and Cultural Office in Kaohsiung,
Taiwan on September 15, 2019;

3. That I hereby authorize MARLON C. LUCIDO, Filipino, of legal age and a resident of
Binangonan Rizal to cause and process the authentication of the
Admission/Acknowledgment of Paternity, Consent to Use Surname and its
corresponding Certificate of Registration before the Department of Foreign Affairs in
Manila; further, hereby giving and granting full powers and authority to do and perform
all and every act requisite and necessary to carry into effect the foregoing authority as
fully and all intents and purposes as I might or could lawfully do if present;

4. That I am willfully executing this Affidavit with Special Power of Attorney to attest the
truthfulness and veracity of the foregoing statements and authority given.
