Submission Contract:
I, the undersigned, have read, understood, and initialed each provision listed below, and I accept and agree
with each of the following:
1. I have read, understood, and complied with all eligibility requirements for the category for which I would
like this film to be considered. __________
2. This film is not subject to litigation nor is threatened by any litigation. __________
3. I am duly authorized to submit this film to the Phoenix Film Festival and its Competition and understand
that the Phoenix Film Festival holds the right to screen my film for Festival screening and promotional
purposes and to extract clips fro this film for promotional purposes. This includes streaming of films on
sites of the Phoenix Film Festival and/or its sponsors and affiliates. __________
4. I understand that the Phoenix Film Festival is not responsible for any type of damage or loss of the print
during the course of the Festival, while the film is in the Festivalʼs possession, or while en route to or from
the Festival, and I release the Phoenix Film Festival personnel from any and all liabilities regarding
damage to or loss of the print while en route to/from the Festival or while in their possession.
