(b). I am applying for the issuance of VEHICLE STICKER(s) described above for the use of NUVALI Boulevard (access to/from NUVALI Gates 2, 3 and 5) and/or ALI
Spine Road Gates.
(c). In cases of violations by my agent, or myself, I hereby acknowledge the authority of Estate Management to revoke this permit, and/or impose penalties/sanctions
without prejudice to legal remedies and claim for damages by Declarant/Association/Estate Management.
(d). I understand and acknowledge that the issuance of the sticker does not carry any acceptance of liability on the part of NUVALI and its authorized personnel, for any
cost or expense from any untoward incident, injury, and damages that may occur arising from/or on the occasion of the operation of the said motor vehicle and/or use of
the sticker, whether within the premises of the road or otherwise.
(e). I bind myself to follow and cause any person acting under its authority or behalf with respect to the motor vehicle subject of this application to comply with all pertinent
rules and regulations.

Supplemental Terms and Conditions

This Permit is hereby granted subject to the conditions stated in the Estate Management Procedures and Guidelines certified through ISO 9001:2008 (Quality), ISO 14001:2004 (Environmental), and OHSAS 18001:2007 (Occupational,
Health, & Safety) Standards, and to the following conditions:

1 Only a NUVALI authorized representative shall post the sticker on the applicant’s vehicle. 9 The Vehicle Sticker also serves as a Road Right Of Way Permit, as applicable.

2 The NUVALI Sticker is NON-TRANSFERABLE to any other vehicle. 10 The NUVALI Estate Management may modify any terms and conditions without prior notice.

3 The issuance of the NUVALI Sticker does not carry any acceptance of liability on the part of NUVALI
Management.
4 All vehicles must comply with traffic rules and regulations being implemented in NUVALI. The speed limit of 60
kph for the NUVALI Boulevard must be observed at all times.
6 The NUVALI Sticker's validity will be advised by the Estate Management thru memorandum and will be subject for
renewal every year.
7 Application forms should be submitted to the Estate Management Office located at the Evoliving Center, NUVALI, Sta.
Rosa, Laguna.
11 The Applicant/Pemittee/Agents agree to fully indemnify and hold harmless the
Declarant/Association/Estate Management, its subsidiaries, joint venture companies, and their respective
officers, employees and agents against any and all claims, demands, actions, losses, liabilities, damages,
and all related cost and expenses, including reasonable legal fees and disbursements and costs of
investigation, litigation, settlement, judgement, interest and penalties, due to or arising from the fault or
negligence of the Applicant/Permittee/Agents, and its representatives, agents, employees, contractors
and/or any party to the work.

8 Processing of application is scheduled from Monday to Friday, from 10:00 AM to 5:00 PM only.
