SUBJECT: Spot Report re-Arrested Person for Violation of Art II, Sec 5 and 11 of RA 9165
REFERENCE: Standard Operating Procedure (SOP).
TDPO: At about 4:35 AM of October 16, 2019 along Lubak St., Sitio Balanti, Brgy. San Isidro, Cainta,
Rizal
SUSPECT: MARLYN FERNANDEZ y SANTOS @LENLEN (Pusher/Arrested/Name submitted to PIB),
Female, 32 years old, born on March 24, 1987 in Cainta Rizal, Single, Tanod of Sitio Balanti, Brgy. San
Isidro, Cainta, Rizal, Highschool Level, Roman Catholic, residing in Blk 1, Lot 21, Sitio Balanti, Brgy. San
Isidro, Cainta Rizal.
NARRATIVE: Investigation disclosed that at about 3:10 AM of October 16, 2019 a confidential informant
came to Cainta MPS and reported about selling of illegal drugs and other illegal activities of a certain
Alyas LENLEN in Lubak St, Brgy San Isidro Cainta Rizal. In response of the given information PLTCOL
ALEX F DELOS SANTOS, Officer In-Charge instructed the operatives to coordinate with PDEA 4A and
said agency gave an authority to operate with control no. 10005-102019-0869. Thereafter, a BUY-BUST
Operation conducted at the given address, wherein PSSg Manny F Fetalver acted as poseur buyer while
PMSg Noel Cabaddu serve as back-up that resulted to the arrest of above mentioned suspect and
confiscation of Five (5) pcs heat-sealed transparent plastic sachet containing white crystalline substance
suspected as shabu marked as MFF-BB, MSF-1, MSF-2, MSF-3 and MSF-4 with total estimated weight
of more or less 0.85 gram, Three (3) pcs. One-hundred peso bills used as Buy Bust money with serial
numbers BB459081, CG424432 and MG296219, and Five(5) pcs. One-Hundred Peso Bill suspected as
drug money.
The arrested suspect was apprised of her constitutional rights and brought to Cainta MPS for
proper investigation/documentation, subsequently the seized and recovered pieces of evidence together
with the suspects will be brought to Rizal Crime Laboratory Office, Taytay, Rizal for appropriate laboratory
examinations.

SUBJECT: Spot Report re-Turned Over of Arrested Person for Carnapping of Two Wheeled Vehicle
TDPO: On or about 3:00 AM of October 16, 2019 Antiporda St., Brgy. Pila-pila, Binangonan, Rizal.
COMPLAINANT: Jimson Mesa y Ceñidoza, 30 years old, born on Februaay 1, 1989 in Morong, Rizal,
with live-in partner, Driver, High School Graduate, Roman Catholic and resident of No. 135 Antiporda St.,
Brgy. Pila-pila, Binangonan, Rizal.
SUSPECT: Manolito Tablando y De Oca, 39 years old, (DOB: February 25, 1980) in Cagayan De Oro,
married, Mechanic, Elementary Level, Roman Catholic, 5’7” in Height, MOL 60 Kg., and resident of Pulo
St., Brgy. Pila-pila, Binangonan, Rizal.
NARRATIVE: On or about 5:00 AM of October 16, 2019, victim together with Arresting Officers turned-
over the suspect for the abovementioned incident. Investigation disclosed that on or about 6:00 AM of
October 15, 2019 victim parked his Kawasaki Barako with sidecar, bearing MV File NO. 0401-
00000946964, color blue, 2019 model, unattended in front of their house and went inside, upon his return
he discovered that the abovementioned tricycle was already missing. Victim immediately reported the
said incident at Brgy. Pila-pila Barangay Hall Binangonan, Rizal for record purpose.
That on or about 3:00 AM October 16, 2019 Barangay Tanod of Brgy. Batingan, Binangonan,
Rizal while conducting patrolling along Bayabas St.,Brgy. Batingan, Binangonan, Rizal saw the suspect
while in the act of dismantling a machine of Kawasaki Barako with sidecar. In that instance, Barangay
Tanods approaches the suspect and asked for the proof of ownership of the vehicle but the latter failed
to present any. Upon questioning the suspect Barangay Tanod learned that the tricycle is a Carnapped
motor vehicle and that the owner is one (1) Jimson Mesa y Ceñidoza of Brgy. Pila-pila, Binangonan,
Rizal. They immediately arrested the suspect and contact the owner that instantly came to Brgy. Batingan,
Binangonan, Rizal. Afterward, the above mentioned suspect was turned-over to this station for
investigation and proper disposition.

TDPO: On October 16, 2019 at about 12:30 AM along Cabrera Road (overlooking) Brgy. San Roque
Antipolo City
SUSPECT: Unidentified
NARRATIVE: This pertains to the Shooting Incident that transpired on October 16, 2019 at about 12:30
AM along Cabrera Road (overlooking), Brgy. San Roque, Antipolo City wherein the victims were identified
as a. Robert Nen y Balaja, male, 21 years old, born on December 6, 1997 in Pasig City, single,
unemployed, and a resident of Brgy. Pinagbuhatan, Matanza, Pasig City (wounded); b. Ronnel Matudan
y Cristobal, male, 33 years old, born on October 7, 1987, married, tricycle driver, and a resident of Brgy.
Pinagbuhatan Matanza, Pasig City (wounded); c. Frank Castro y Cruz, 21 years old, born on August
10, 1998 in Pasig City, single, unemployed, and a resident of Brgy. Pinagbuhatan, Pasig City (wounded);
and d. Medellin Dantoc y Villanueva, male, 28 years old, born on April 12, 1991, single, salesman, and
a resident of Brgy. Pinagbuhatan, Pasig City (wounded), while the suspects were still unidentified.
Please be informed that follow-up investigation conducted by the Investigators on Case of Antipolo
City Police Station revealed that another two (2) victims were identified: a. Wilson Ramos y Espiritu,
male, 19 years old, born on May 7, 2000 in San Pablo Laguna, single, self-employed, and a resident of
Victoria St., Brgy. San Juan, Taytay, Rizal; and b. Joshua Lopez y Santos, male, 19 years old, born on
July 13, 2000 in Taytay Rizal, single, unemployed, and a resident of #79 Maria Clara St., Brgy. Ana,
Taytay, Rizal both sustained gunshot wounds (GSW).
Further, two (2) vehicles were also hit by bullet described as a. Toyota Hi-Ace, color white,
engine number 2KDSA724859, Plate Number AKA 4762, registered to Bryan Anthony E Bequiso of Pasig
City and b. Mitsubishi Adventure, with engine number 4G63AC3033, Plate Number XSD583 registered
to Maria Minette Robles Navarro of Pasig City and were now under the custody of SOCO Rizal PPO for
Trajectory Test.
SOCO Team led by MAJ JOSEPH P TORCITA conducted crime scene processing at the
place of incident. Recovered pieces of evidence from the crime scene were: thirty-six (36) fired cartridge
cases (FCC); two (2) cartridge; two (2) fragment; two (2) deformed bullets (DB); three (3) jacketed bullet;
(JB) and one (1) lead slug with a total of forty six (46).
Antipolo CPS are still conducting follow-up investigation for possible identification of the
vehicle used and arrest of the suspects. Any development relative thereto will be forwarded to that Office
as soon as obtained.

TDPO: At about 5:50 AM of October 16, 2019 inside the Basketball Court of Sitio Victoria, Brgy. San
Juan, Cainta, Rizal
SUSPECT: JHONFER BRIX ADOC y BERNARDINO@ADOC (Arrested/User/not listed), male, 23 years
old, born on May 23, 1998 in Cainta Rizal, Single, College Student, Jobless, Roman Catholic, residing in
054, Sitio Victoria, Brgy. San Juan, Cainta, Rizal.
NARRATIVE: Investigation disclosed that on or about 5:40 AM October 16, 2019, Tactical Operation
Center (TOC) of Cainta MPS received a telephone call from a concerned citizen that reported about a
malefactor using illegal drugs inside the Basketball Court of Sitio Victoria, Brgy. San Juan, Cainta, Rizal.
In response, duty TOC immediately informed the Intel/DET operatives who were conducting
validation and monitoring against all forms of illegal activities at said address. Upon arrival, Arresting
Officers saw the above mentioned suspect on act sniffing/using of illegal drugs(marijuana) that resulted
to his arrest wherein Pat Jim Albert Rabuya also able to confiscate Two(2) pcs Zip lock plastic sachet
containing dried leaves and stalks of suspected Marijuana with estimated weight of more/less 6 grams
marked as JBA-1 and JBA-2, One(1) pc Improvised Tube Pipe containing traces of suspected marijuana
marked as JBA-3, One(1) pc Disposable Lighter marked as JBA-4, One(1) pc Square Cannister color
red marked as JBA-5.
The arrested suspect was apprised of his constitutional rights and brought to this station for proper
investigation/ documentation, subsequently the seized and recovered pieces of evidence together with
the suspects will be brought to Rizal Crime Laboratory Office, Taytay, Rizal for appropriate laboratory
examinations.

Caunan of RTC Branch 75, San Mateo, Rizal dated March 23, 2015; and
b. LOI 03-11 “MANHUNT CHARLIE”.
TDPO: At about 09:30 AM of October 16, 2019 in Brgy. Burgos, Rodriguez, Rizal.
SUSPECT: Karla Pineda y Camilet, 31 years old, born March 15, 1988 in Marikina City, married, jobless
and residing in B3 L7 Phase 1 Dela Costa Homes V, Burgos, Rodriguez, Rizal.
NARRATIVE: This pertains to the conduct of operation against Wanted Person by elements of Warrant
and Subpoena Section of Rodrigiguez MPS at about 09:30 AM of October 16, 2019 in Brgy. Burgos,
Rodriguez, Rizal which resulted in the arrest of Karla Pineda y Camilet. The accused was arrested by
virtue of Warrant of Arrest for the crime of Estafa, issued by the aforesaid Honorable Judge with bail
recommended of Php16,000.00.
The said accused was apprised of her Constitutional Rights upon arrest and was brought to
Casimiro Ynares Sr Memorial Hospital for Medico Legal Examination. Likewise, she was subjected for
booking and arrest procedures and presently detained at the custodial facility of Rodriguez MPS.

SUSPECT: Ronaldo Aviles y Gomez, male, 54 years old, married, born on February 24, 1965 in
Cardona, Rizal, jobless, Catholic, and residing in JP Rizal Brgy. Subay, Cardona, Rizal (At-Large).
WITNESS: Keyzer Antonil y Aviles, male, 23 years old, single, born on January 16, 1997 in Cardona,
Rizal, Utility Support, Catholic, and presently residing at JP Rizal., Brgy. Subay, Cardona, Rizal.
NARRATIVE: On October 16, 2019 at about 8:10 AM, said complainant and witness personally appeared
Cardona MPS and presented a Certification to File Action issued by Hon. Restituto R. Loyola, Brgy.
Captain of Brgy. Subay, Cardona, Rizal dated September 30, 2019 to file a complaint against the above
mentioned suspect.
Investigation conducted by Cardona MPS disclosed that on the said DTPO, while the victim/
complainant was inside of his house, wherein he heard her nephew crying from the outside and
immediately went to and approached her. And there, he saw Ronaldo yelling at her nephew and asked
why he was yelling at the kid. At that point the suspect threatened him and said “putang ina mo lumayas
kayo sa bahay ninyo at pag di kayo umalis dyan ay papatayin ko kayo”. That both parties involved were
called at barangay Hall of Brgy. Subay, Cardona, Rizal for three (3) times for possible settlement but fail
to come to an agreement prompted them to report the incident for investigation and proper disposition.

Single, born on January 17, 1983 in Pasig City, native of Rizal and residing in Zone 4, Sitio Halang, Bgy.
San Isidro, Cainta, Rizal.
NARRATIVE: At about 11:00AM of October 16, 2019 personnel Cainta MPS conducted operation against
Most Wanted Person at Brgy. San Isidro, Cainta, Rizal that resulted to the arrest of the above mentioned
Wanted Person by virtue of WOA issued by PRESIDING JUDGE MARY JOSEPHINE P. LAZARO of
RTC, 4th JUDICIAL REGION, BRANCH 74, ANTIPOLO CITY dated July 15, 2015 for the crime of
ROBBERY with C.C Nr/s. 14-49635 with recommended bail of Php 100, 000.00. Accused is now
presently detained at Cainta Custodial Facility while the copy of the Warrant of Arrest will be returned to
the court of origin for the issuance of Commitment Order.

NARRATIVE: At about 9:45 AM of October 15, 2019 PSSg Clemente Allauigan and PSSg Paul Nolasco
together with the witness one Rovielyn Gain y Lardizabal, 46 y/o, born on August 20, 1973, with live-in
partner, Sewer, and residing in Sitio Siwang, Brgy. San Juan, Taytay, Rizal brought to Taytay MPS the
arrested suspects to file a complaint.
Investigation disclosed that, while the Arresting Officers conducting patrolling with area of
responsibility, witness approached and reported that, accordingly, suspects 1and 2 selling stolen tricycle
amounting to php7,000.00 prompting the Police Officers to validate said report. Upon verification and
asking documents, suspects failed to present any proof of ownership relative to the tricycle, hence,
suspects1 and 2 brought to Taytay MPS for investigation leading to the discovery that the Honda tricycle
with MV file 1308-00000447047 was forcibly stolen, carted away while parked unattended.
Pertinent documents was being prepared for referral of the case to the Prosecutors office against
the suspects.

given information, Officer-in-Charge PCPT ROLANDO S DESUYO instructed the MDET operatives to
immediately validate and coordinate to PDEA 4A thru electronic mail (email). Upon receipt of PDEA
Coordination with Control Number 10005-092019-0878, MDET operatives were dispatched on board
private vehicles to conduct buy-bust operation thru intelligence driven police operation with the assistance
of the confidential informant.
That on the said DTPO, Anti-illegal drugs operation (Buy-Bust) was conducted which resulted in
the arrest of the suspect after the latter sold one piece small HSTPS suspected shabu and marked with
RGL to the police officer who acted as poseur buyer in exchange with the buy-bust money five hundred
peso bill with serial No. LK900284 and while being frisked, another two (2) pieces of HSTPS containing
white crystalline substance suspected shabu and marked with RGL-1 ang RGL-2 and the buy bust
money one piece of five hundred peso bill (Php.500.00) marked with JMPS.
Arrested suspect together with the pieces of evidence recovered were brought to PNP Rizal Crime
Laboratory Office for Drug Test and chemical analysis.

NARRATIVE: On or about 8:40 PM of October 16, 2019, a confidential informant personally appeared to
Angono MPS and reported that a certain alias “JOSEPH” was engaged in selling illegal drugs along E.
Rodriguez St., Brgy. San Vicente Angono, Rizal. Upon receipt of the information and after having been
given a control number 10005-102019-0898 from PDEA 4A, D.E.T operatives of this PS led by PCMS

NARRATIVE: At about 11:00 PM of October 16, 2019, Pat Diego Badang turn over Cainta MPS the said
suspect for proper disposition of the case.
Investigation disclosed that on or about 8:00 PM October 16, 2019, said victim was crossing along
Felix Ave. from west going to the other side of the road, while the suspect was driving his Honda Civic
Model 96, colored blue with plate Nr UJH 965 traversing along the said road heading towards north came
from south direction, positioned at the outer lane and upon reaching the place of occurrence, the victim
was accidentally hit and bumped thus inflicting injuries in the head. Victim was rushed at One Cainta
Hospital by ambulance for medical attention while the suspect was turned over to Pat Diego Badang by
Brgy Tanod of San Isidro and at about 11:00 pm of October 16, 2019, suspect was brought to Cainta
MPS for further investigation and disposition.

operatives of Rizal PPO.
SUSPECT: RAUL CORTEZ y VIRI aka BORDO, male, 44 years old, married, Laiason Officer, college
level, catholic, born on December 3, 1975 in Antipolo City and residing in Samson Compound, C. Lawis
Extension, Brgy. San Luis, Antipolo City. (Arrested/Pusher/Listed)
NARRATIVE: At about 11:30 PM of October 16, 2019, PSMS Manimtim and PSSg Adoc, both
operatives of Provincial Intelligence Branch (PIB) returned PIB, Taytay Rizal and brought the above-
mentioned suspect for Violation of RA 9165 for proper disposition.
Investigation conducted by Provincial Intelligence branch disclosed that about 6:30 PM of October
16, 2019, PSMS Manimtim received a call from a confidential informant that there is rampant selling of
illegal drugs known as SHABU in Samson Compound, C. Lawis Extension, Brgy. San Luis, Antipolo City
in which certain “ALYAS BORDO” was behind on said illegal activities. In response of the given
information, PCPT JUAN CARLO A PORCIUNCULA, Deputy, PIB under direct supervision of PLTCOL
NOEL A CAMPOS, Chief, PIB organized strategic planning and briefing wherein PSMS Manimtim is
designated as poseur-buyer. Thereafter, Anti-Illegal Drug Buy-Bust Operation was conducted at the given
address with joint effort of PDEA RIZAL and ANTIPOLO CPS in coordination with PDEA CALABARZON
Pre-Ops Control No.10005-102019-0893 and Authority to Operate that resulted to the arrest of the
aforementioned suspect and confiscation of the aforementioned pieces of evidence. All
seized/confiscated plastic sachets has combine estimated weight of more or less 3 grams.
The arrested person was informed the Miranda Doctrine and Anti-Torture Law. Subsequently,
actual physical inventory and photograph of seized/confiscated pieces of evidence conducted at the place
of the incident witnessed by Barangay Kagawad and Media Representative in relation to SECTION 21
OF RA 9165. The aforesaid suspect together with confiscated evidence were brought to Rizal Provincial
Crime Laboratory Office, Hilltop, Taytay, Rizal for appropriate laboratory examinations.

1. Jerissa Francisco y Tobes, 21 years old, single, part time job in Valley Golf Canteen, born on
June 15, 1997 in Cainta, Rizal, a resident of Sitio Burol, Brgy. May-Iba, Teresa, Rizal (Deceased),
driver of Rusi Motorcycle without plate number; and
2. Aira Tuyay y Asuelo, 22 years old, single, Valley Golf Canteen helper, born on November 2,
1996 in Cainta, Rizal, a resident of Sitio Uulan Valley Golf, Brgy. Dela Paz, Antipolo City
(Backrider/Confined at Cainta Hospital).
SUSPECT: John Carey y Belen, 68 years old, born on August 8, 1951, a resident of # 2 Acacia Lane
Palmera Heights, Brgy. San Juan, Cainta, Rizal (At-large), driver of Toyota Tamaraw with plate number
TTT 284 (Impounded).
NARRATIVE: At about 12:05 AM of October 17, 2019, a telephone call from Valley Golf Security Officer
Jeffrey Roderin Anoba informing the above victims were brought to Cainta Hospital for the above incident.
Acting of the report, Officer on Case with mobile crew were dispatched to conduct investigation.
Investigation shows that victim # 1 was a driver of the motorcycle together with backride/victim # 2, they
moving uphill going to valley golf course direction. Upon reaching in the narrow curved lane, suspect
driving the Toyota Tamaraw with plate number TTT 288 in opposite lane moving downhill with speeding
and allegedly encroached the lane of the victims. Due to the incident, victims sustained injuries in different
parts of their bodies and was brought to TACANO Cainta Hospital. Unfortunately, victim # 1 was declared
dead on arrival while treated thereat while victim # 2 was confined at the same hospital. The suspect
driver was able to escape after leaving his driver’s license to the security guard. Follow up Operation
conducted by Antipolo CPS for the arrest of the suspect driver but he was not found on his given address.

Damaged Property: Mitsubishi Mirage, Red and with Conduction Sticker number NK 8613.
SUSPECTS:
1. AARON VINCE FLORES y GROSPE, 19 years old, born on March 3, 2000 and a resident of A.
Bonifacio Ave. Brgy. Sto. Domingo Cainta, Rizal (Malicious Mischief and victim of Physical Injury)
2. EDMOND BAISAS y VILLAREAL, 21 years old, born on August 17, 1998 and a resident of 013
Vicencio St., Felix Subd. Brgy. San Andres Cainta, Rizal.

NARRATIVE: At about 12:30 AM of October 17, 2019, suspect AARON VINCE FLORES was brought at
Cainta MPS by Tanod of Brgy. San Andres, Cainta, Rizal. It appears that suspect had a drinking spree
with his friends somewhere in Cainta. At about 11:00 PM of October 16, 2019 he decided to go home.
Apparently, he passed by at Felix Subd. and while walking, he suddenly punched victim’s vehicle while it
was parked along the street breaking the rear windshield. Bystanders in the area saw the incident and
mauled the suspect. After several minutes, Tanod from Brgy. San Andres arrived at the area and arrested
the suspect. The suspect was brought at Cainta Hospital for treatment and later on brought at Cainta
MPS. The case was then turned over to PSSg Bidaure for investigation and the suspect Aaron Vince
Flores was brought at Cainta Custodial Facility however, suddenly, suspect Edmond Baisas appeared
and punched Aaron Vince Flores while investigator on case was turning over the latter. He (Edmond
Villareal) was immediately placed under arrest, brought at Cainta Hospital for medico legal examination
and was turned over at Cainta Custodial Facility.
Copy of CCTV camera footage owner installed near the place of incident was requested for possible
identification of the suspects. Operatives of this station are still conducting follow up operation for possible
identification and arrest of the suspects.

Rodolfo Dela Cruz y Rivera, all of legal age, currently designated as Barangay Tanods of Brgy. San
Roque, Angono, Rizal.
SUSPECT: Noelito Bautista y Navilon, 28 years old, born on November 14, 1990 in Pandacan, Manila,
single, security guard, 5”5’ in height, 80 Kg., with tattoo on right shoulder and on both hands, and a
resident of 0045 El Bote St., Brgy. Darangan, Binangonan, Rizal. (Arrested);
NARRATIVE: Investigation disclosed that on the said TDPO, above-mentioned victims/Arresting
Officers received a complaint from a concerned citizen about the suspect who was under the influence of
liquor and continuously shouting to any individual he saw while breaking bottles of beer on the road. His
unruly behaviour causing fear and disturbance to the residents in the area. Thereafter, above-mentioned
victims/Arresting Officers immediately responded to the scene and upon arrival thereat, they introduce
themselves to the suspect as Barangay Tanods to pacify him, however instead of submitting himself
peacefully, said suspect shouted defamatory words by repeatedly saying “MGA PUTANG INA NYO!
DYAN LANG KAYO!”. When above-mentioned suspect about to throw bottle of bear towards them, they
immediately reacted to arrest him, but said suspect resisted until they restraint him and effect the arrest.
The arrested suspect was apprised of his Constitutional Rights in dialect known to him but opted
to remain silent in the absence of his chosen counsel.
