COMMUNICATION ROLEPLAY IN EVERY NURSING STAGE OF NURSING IN

FULFILING LIQUID, ELECTROLY AND BLOOD NEEDS

Prologue
There a patient who was treated at Dr. General Hospital Setiawan, precisely in Melati's
room named Rosa, she was 35 years old, she got anemia. The patient looked weak, pale and the
results her Hb laboratory tests was less than normal.
1. Introduction phase
(One morning nurse came into the room)
Nurse I : ”Good morning, hello my name is Zalsha , I have another assignment in this
room, from 07.00 till 14.00 this afternoon. I am nurse who is responsible for
caring for you, what is your name?”
Patient : “My name is Karol Hapipah”
Nurse 1 : “How old are you ?”.
Patient : “40 years old”
Nurse 1 : “ Let me check your id number please? What you Occupation?”
Patient : “ I from West Java”
Nurse 1 : “Now I Wanna Check your vital sign, are blood pressure, pulse, respiration rate,
temperature and saturation”
Patient : “Does it hurt?”
Nurse 1 : “No, just calm down”
After doing the vital sign
 Nurse 1 : “ The results obtained the examinations of vital sign are, your blood pressure is
90/60, your pulse is 80 per minute, your respiration rate is 24 per minutes,
your temperature is 36,5 C, and your saturation is 97%
Patient : “ Is it normal ?”
Nurse I : “ Still abnormal, but can still be normallized with another treatment”
Nurse I : “Who I accompanied you today ?”
Patient : “ My daughter”
Nurse I : “What is your daughter’s name, where is she?”
Patient : “Shakila, but she goes out looking for food”
Nurse : “Is there any question that you need?”
Patient : “No.”
Nurse 1 : “If that something you wanna ask, please press the button over there, excuse
me.”

2. Assessment Phase
Nurse II : “Mrs. Karol how are you today? My name is Windi. I will take care of you
today. How do you feel now?”
Patient : “Now, I feel better, but at other times, I feel headache and weak”.
Nurse II : “How often do you feel weak and headache?”
Patient : “Usually I always feel so weak in late afternoon or when I feel hungry”
Nurse II : “How long did you feel it ?”
Patient : “It's been a long time. And lately more often”
Nurse II : “Do you have a history of certain diseases?”
Patient : “I have an anemia”
Nurse II : “Does the other family have a history of the same disease?”
Patient : “My mother has the same disease”
Nurse II : “Alright then I will consult about your complaint to the doctor in charge. I will
return at lunch time”
Patient : “Ok, I see”
3. Diagnostic and Planning Stage
After the nurse talked to the patient, the nurse came to the doctor's office
Nurse II : "(tok tok) excuse me doc"
Doctor : "Yes, please sit down, is there anything that you need?"
Nurse II : “There is a patient named Karol Hapipah, she is 35 years old coming to the
hospital with complaints of weakness and slight dizziness, plus she has defecated
four times a day. And laboratory test of Hb is 8 g/dl.”
Doctor : "Well I will go to the patient's room to check her condition."
Nurse II : "Ok doc, please". (nurses accompanied the doctor).
After being examined, the doctor asked the nurse to do the installation normal saline
infusion to Mrs. Karol
Doctor : "Well, then just give the procedure of infusion to increase the fluid needs."
Nurse II : "Ok, I will immediately take these actions".
The nurse immediately prepared tools to infus installation
Nurse II : Excuse me, I am here to do the infusion because you must immediately get fluid
fulfillment, so your fluid is fulfilled. Before I do this, is there something you want
to ask? "
Patient : "Well, there is, but first I want to ask to you, is the infusion dangerous or
unsuspecting?, then is there any side effects of it?"
Nurse II : "There is not side effects, ma'am, but when I put the needle in it, it will hurt a
little, so you must take a deep breath, don't pull your hand away, do you
understand, ma’am?”
Patient : "Yes, I understand”.
A few hours later the doctor entered back to check on the patient's condition.
Doctor : "Good afternoon".
Patient : "Afternoon doc".
Doctor : "My name is Hani, I am doctor in charge of you here, I will check your
condition”
Daughter : "Doc, what will you do to my mother, my mother had been stabbed, she’s so pity,
don't do anything anymore".
Doctor : "Calm down, your mother will be fine. I will do the best for her ".
 Patient : "Yes, you don't worry, the doctor is experienced, surely he will give the best
treatment."
Daughter : "Yes ma'am, I don’t want you to get worse, I want you to recover quickly".
Doctor : "Well, now how is your condition? Do you feel better or are there any
complaints?"
Patient : "I still often feel dizzy doc, the dizziness is’nt gone, my eyesight is blurry".
Doctor : "Let me check you”
After the doctor checked her condition, she got a blood transfusion cause of the blood
deficiency
Doctor : “Now I have finished checking it, it turns out you must immediately have a
blood transfusion, because you get lower blood. Later there will be a nurse who
will give you a blood transfusion, don’t worry, we’ll do our best.”
Patient : Alright doc, thanks a lot ".
4. Implementation Phase
After that the doctor also gave the task of blood transfusion to nurse 1 and 3, and they are
rushed to the room to provide blood transfusion.
Nurse III : "Good Afternoon ma’am".
Patient : "Good Afternoon too"
Nurse III : "Yes ma'am, I am nurse Dina, here I am helping nurse Windi to do blood
transfusion to you who had previously been instructed by doctor Hani".
Daughter : "My mother so pity, are you going to stabbed her again?”
Nurse III : "Your mother will be given extra blood, because she gets of dizziness and her
Hb is so low, don't worry, we will provide the best service for your mother”
Daughter : (glum looking)
Nurse III : It's okay, it's also good for your mother, so that you can get back to your
activities well, even though it's still in the recovery phase. "
Daughter : "Are there any side effects?”
Nurse III : "Nothing"
Nurse III : "Yes, ma'am, let's just start, ma'am, so that the implementation will be faster and
better, later I will do the action".
Patient : “Ok”
 Nurse III : "Wait for a minute, ma'am, I will prepare the tools first".
Patient : "Yes".
Nurses also performed blood transfusion actions
Nurse III : "Okay Ma'am, I'm starting now, Ma'am, relax, calm down Ma'am, I'm just
replacing the IV fluid with a blood bag."
Nurse III : "Ma'am, I have completed the blood transfusion installation. How do you feel?"
Patient. : "I'm fine, the flow of blood feels slow."
Nurse III : "Yes ma'am, just relax, there are no side effects, so you don't need to worry,
Ma'am.”
Patient : "Oh, I see, thank you."
Nurse III : "Yes, ma'am, okay, is there any question?”
Patient :"Nothing"
5. Evaluation and Termination Phase
3 days later the doctor was accompanied by a nurse to check the patient's condition again
and the patient's condition had begun to improve.
Doctor : "Good evening"
Patient : "Good evening doc."
Doctor : "How do you feel now?”
Patient : "It has begun to get better and i feel no longer dizzy."
Doctor : "Waaah, good for you, thanks God, you may go to your home now".
Patient : "Yes thanks doc, waaahh it turns out now I can go home doc?”
Doctor : "Yes ma'am, because after I check it turns out that the fluid needed have been
fulfilled, and later at 10 minutes the nurse will enter the room to release
your infusion,”
Patient : "Ok doc, thanks"
A few minutes later the nurses came to release the infusion and give the health education to
her about anemia.
Health Nurse : "Good afternoon ma'am, today, my name is Vira, I will release the infusion
first".
Patient : "Yes". (nurse released the IV according to the procedur)
Health Nurse : “According to doctor instruction you may go to your home alright, how do you
feel?”
Daughter : “Oh yes, iam going to prepare it.”
Health Nurse : “I want to tell you about anemia’s health education, I have a leaflet and i will
“give it to you. Please read
Health Nurse : "When you get home try to get the iron intake of foods such as spinach,
cabbage, and kale Chicken liver, eating unskin meat, drinking a large
amount of water a day minimum 8 glasses, getting sufficient rest and taking
medication on a regular basis.”
Patient : "Yes, later at home I will take care of my diet well and get enough rest. Thank
you for serving me well "
Health Nurse : “For you shakila, please take care of your mother to always stay healthy.”
Daughter : “I will give more attention to my mother, thank you.”
Health Nurse : "It is our duty as a medical staff to serve patients well."
Finally Rosa's mother had recovered, and she soon returned home together with her son.

Conclusion
Nurses can do their job and provide more knowledge about the importance of fluid intake to
patients because of communication and collaboration with the patient is going well.
