I. ADMINISTRATION

1. ADMINISTRATIVE SUPPORT SYSTEMS

1.1 Barangay Hall


 For Barangay-owned structure and lot– MOVs: Certificate of Land Title/Deed of
Donation

1.4 Barangay Map


 Displayed within the barangay hall – full points (0.5)
 Kept on barangay records – 0.3 point

1.6 Visible Signage or signboards


 i.e. This way to Barangay Hall/Day Care Center signage

1.7 Pictures of Political Leaders


 should be the updated - from President-Governor-Mayor-Punong Barangay

1.9 Functional and Potable Water System – identify the source of potable water

1.17 Presence of VAWC Desk Officer – MOVs: Copy of any similar issuance i.e. Executive
Order/Resolution/Designation Order, Copy of Assigned Schedule of designated VAWC
Desk Officers

1.19 Presence of Volunteer Workers – MOVs: Copy of any similar issuance i.e.
EO/Designation Order, Copy of Assigned Schedule and conduct brief interview to assess
their knowledge on their assigned tasks/roles

3.4 Peace and Order


- Functionality= look into structure, meetings, policies, budget, and accomplishments
- BADAC Meetings= from 2nd Semester 2016 to 1st Semester 2017 (at least once a month)
- Conduct of IEC on illegal drug demand reduction = from 2nd Semester 2016 to 1st Semester
2017
- Presence of plan to improve peace and order and public safety in the barangay= formulated
2017

2. TRANSPARENCY AND ACCOUNTABILITY

2.1 Implementation of ARTA (RA 9485)


 Ordinance adopting Citizens Charter – baseline year 2013 (consider also
ordinance on the reorganization/strengthening of ARTA)
 Presence of Feedback Mechanisms –
- Complaint or Feedback Forms – review record book and track course of
action/s on complaints/feedback received (Jan-Dec 2015)
 Special lane for senior citizens and PWDs – presence of signage/signboard for
priority lane
2.3 Full Disclosure Policy
 Posting of List of Notices of Award – if none, require copy of Certification of No
Bidding
2.6 CSOs/POs Mandatory Membership of ¼ of total membership to councils (BDC,
BPOC) – MOV: copy of EO re organization of councils

3. BASIC SERVICES AND FACILITIES

3.1 Social
 Presence of functional KP- MOVs: EO organizing Lupon; updated records of LT
meetings, complaints/cases, and activities undertaken (trainings, barangay/field
visits, etc.)
 Presence of Health/Hygiene Programs – should be y2nd Sem 2016 and 1st Sem 2017
accomplishments; MOVs: documentation reports, plans
 BDRRM OpCen – MOVs: corner/office with basic facilities
 Plaza/Park/Recreation Area – basketball court is considered
 Barangay Vehicle: can be bicycle, scooter, multi-cab
 Barangay Tanod Ronda- copy of order or schedule of ronda
 Sports/Cultural Activities – need to provide documentation
 Water Supply System- provided or managed or maintained by the barangay

3.2 Economic
 Agricultural Support Services
o Distribution of Seedlings: presence of list of recipients
o Multi-purpose/drying pavement: rental services should be supported
by an ordinance; if availment is free, no need for ordinance
o Livelihood Programs: documentation, interview
3.3 Environmental
 Presence of waste receptacles: check garbage bins (follow segregation:
nabubulok/di-nabubulok/recyclable)
 SWMP- RA 9003 advocacy and waste management policies (simple
format/matrix is considered)
 Tree planting, beautification, clean up drive – require documentation

II. RECORDS MANAGEMENT AND FILING SYSTEM

Baseline:
a. 201 Files of Barangay Officials and Employees: updated – basis is elected officials from
the 2013 Barangay Election
b. SALN – as of year 2016
c. Oath of Office of elective and appointive barangay officials and employees: year 2013
d. Updated Registry of Barangay Inhabitants- as of 1st Sem 2017
e. List of Precincts and Registered Voters: as of 2013
f. Files of Inventory of Barangay Properties and Facilities- as of 2016
g. Files of Minutes of Session- coverage is year 2nd Sem 2016 and 1st Sem 2017
h. BDC composition must include BDRRMC as sub-committee
i. BADAC- BPATs and other groups must be present

III. BARANGAY LEGISLATION


1. Adopted Internal Rules of Procedure- adopted by year 2013

IV. FISCAL MANAGEMENT


1. Duly approved Barangay Development Plan: matrix is considered
2. Duly approved Barangay Budget- year 2017
3. AIP and APP: year 2017
4. BAC Meetings – 2nd Sem 2016 to 1st Sem 2017
- Items to Bid: if none, need to provide certification as MOV
5. Project Documents- MOV: Physical and Financial Accomplishment report (matrix
form considered- name of project, cost, location, funding source, date completed) or
Certificate of Turn Over (if project came from National)
6. Fiscal Administration Best Practices/Innovations: regardless of number, give full
points
