FOR POWERHOUSE

1.1. CONSTRUCTION ACTIVITIES:

1.1.1. Excavation: Excavation has been completed.

1.2. OTHER ACTIVITIES:

1.2.1. Curing of poured Concrete is in progress on different locations/parts is in progress on
Switchyards at Erection Bay-I & Erection Bay-II, Unit #1, Unit #2, Unit #3 and Unit #4.
1.2.2. Fixing of reinforcing steel bars and Formwork on different locations/parts is in progress
on Switchyards at Erection Bay-I & -II, Unit #1, #2, #3 and #4.
1.2.3. Poured 338m3 Concrete on Unit #1 Block U1-II-8 at El. 396.97 to 398.47 from 08.10.2019
at 21:30pm to 10.10.2019 at 04:30am.
1.2.4. Poured 353m3 Concrete on Switchyard Unit #3 Block SU3-9 at El. 416.3 to 419.0 from
08.10.2019 at 23:00pm to 10.10.2019 at 06:00am.

1.3. ISSUES:
1.3.1 Safety measures are very poor.
1.3.2 Housekeeping is poor.
1.3.3 CVI-PH-009 issued, regarding poor ply wood Formwork. Picture 2.7 below.
1.3.4 CVI-PH-010 issued, regarding Cold Joint in Concrete. Picture 2.8 below.
1.3.5 CVI-PH-011 issued, regarding unevenness on surface of Concrete. Picture 2.9
below.

1.4. INSPECTIONS:
1.4.1. 09.10.2019: Inspected reinforcing steel bars and preparation of Construction Joint on
Unit #4 Block U4-S-UT-3 at El. 366.2 to 368.2 and allowed for pouring of second stage
Concrete around elbow portion of steel liner Draft Tube subject to the clearing of all
tests results of concrete of all prior pours below the mentioned elevation of the same
structural element as well as subject to the clearing of all the RFIs of the same location
by the concerned electrical and mechanical staff of SSS-JV. Refer to approved drawings
vide letter number SSS/5065056/CC/1.10.02/4252 dated 23rd of May 2019.
1.4.2. 10.09.2019: Inspected reinforcing steel bars and green-cut/chipping on Switchyard
Erection Bay-II Block SEBII-17 at El. 427.97 to 430.97 and allowed for pouring of Concrete
subject to the clearing of all tests results of concrete of all prior pours below the
mentioned elevation of the same structural element as well as subject to the clearing of

all the RFIs of the same location by the concerned electrical and mechanical staff of SSS-
JV. Refer to approved drawings vide letter number SSS/5065056/CC/1.10.02/4300
dated 14th of June 2019.
1.4.3. 10.09.2019: Inspected 2 times reinforcing steel bars, Formwork and green-
cut/chipping/preparation of Construction Joint on Upstream Auxiliary Powerhouse Unit
#2 Block U2-III-1 at El. 390.97 to 394.47 and allowed for pouring of Concrete subject to
the clearing of all tests results of concrete of all prior pours below the mentioned
elevation of the same structural element as well as subject to the clearing of all the RFIs
of the same location by the concerned electrical and mechanical staff of SSS-JV. Refer to
approved drawings vide letter number SSS/5065056/CC/1.10.02/4092 dated 24th of
April 2019.
1.4.4. 10.09.2019: Inspected reinforcing steel bars, Formwork and green-
cut/chipping/preparation of Construction Joint on Unit #3 Block U3-II-22-1 at El. 408.9 to
411.83 and allowed for pouring of Concrete subject to the clearing of all tests results of
concrete of all prior pours below the mentioned elevation of the same structural
element as well as subject to the clearing of all the RFIs of the same location by the
concerned electrical and mechanical staff of SSS-JV. Refer to approved drawings vide
letter number SSS/5065056/CC/1.10.02/4227 dated 20th of May 2019 and
SSS/5065056/CC/1.10.02/4092 dated 24th of April 2019.
