BI-10-02399-0006 Yanbu Distribution Hub Project 17th May 2018 during routine surveillance at location

Utility Power Sub-station it has been noticed that contractor proceed the excavation work activity
without controlled and IFC drawing details. As per SAES-S-70 ref.3 mentioned that all referenced
specifications, standards, codes, forms, drawings and similar material (including all revisions, addenda
and supplements) shall be of the latest issue unless stated otherwise. Construction team shall
investigate the nonconformity and provide root cause, Correction and Corrective action for this violation

During Site surviellance it was observed that FBE coated rebars not proper stored as per Saudi Aramco
Standard SAES-Q-001 located at E&M building. This is a clear violation, construction is advised to
proposed preventive action.

BI-10-02399-0006 Yanbu Distribution Hub Project 7 June 2018 during routine surveillance at location
Utility Power Sub-station, Receiving Power Substation, Engineering & Maintenance Bldg. / Musalla
Mosque it has been noticed that FBE Coated Bars has been delivered. During verification some missing
documents found not available like Controlled and IFC drawing details of Rebar Reinforcing Bar Schedule
as per reference of SAES-Q-001 sec.7.18 para.7.18.1 & 7.18.2. Construction team shall investigate the
nonconformity and provide root cause, Correction and Corrective action for this violation

During site surveillance on June 27, 2018, it was observed that the

construction team is already started installing formworks for pipe sleepers foundation. It was noted that
formwork sheeting for pipe sleepers SLP-L-68, SLP-L-75, SLP-L-78 are old forms and damaged ( As per
photo attached ) and not meeting with the requirements of SAES-Q-001 sec.3.2 ref.to ACI 347 sec.5.5.1
where stated that re used forms should not be damaged, distorted, disassembled, or other wise unable
to perform as design.

During site surveillance on July 08, 2018, It has been observed that

the Construction team start installation of rebar for Pipe Sleeper from SLP-L-39 to SLP-L-76, and length
of vertical Rebar R3 for Pedestal indicate as SLP-X-XXX-205 in IFC XA-775558-008-F3 is shorter than
required length up to 30mm to 40mm. It is not complying

with IFC. Fabrication of rebar is not as per IFC XA-775558-008-F3

During routine site surveillance on July 12, 2018 at Pipe Sleeper

SLP-L-69 to SLP-L-76 it has been found that surface of Epoxy Coated rebar are contaminated with
laitance over rebar and foreign matters which may affect concrete bonding. It is nonconformance with
SAES-Q-001 referring to ACI 318 Sect. 7.4.1 which states that All time concrete is placed reinforcement
shall be free from mud oil and other nonmetallic coating that decrease bond.

During routine surveillance on July 12, 2018 at Pipe Sleepers, it wasobserved that FBE coated rebars
storage and preservation not as per Saudi Aramco SAES-Q-001. This is clear standard violation.
Contractor is advised to propose preventive action otherwise it be escalated to SA standard violation.

During routine site surveillance at utility sub-station on July 21, 2018 it was found that construction
team disturb and remove from some places formworks for lean concrete and fail to maintain on its
position. That was inspected and accepted by SAPID against Civil RFI C-038 with EPM no.1400005990. It
is noncompliance of Schedule Q attach. IV sec. 3.16.5. Where states that submittal of any inspection and
testing report directly to company shall not relieve the contractor from its responsibilities under this
Schedule Q.

During concrete pouring of sleepers foundation SLP-L-26 and SLP-L-27, it was found that worker are
Sprinkling water over fresh concrete for finishing. As per SAES-Q-001 sec.3.2 refer to ACI-301 sec.5.3.4
there is no privilege to add water during concrete finishing operation. In other side it is nonconformity
with ACI-301-11 sec.4.3.2.1 where stated do not add water to achieve flow able concrete. Root cause:
Masons were not familiar with standard requirements.

During routine surveillance on 21st July 2018 at Pipe Sleepers foundation SLP-L-69 to SLP-L-76, it was
found out that plastic shrinkage cracks appears on foundation top surface. Cracks resulted from in
adequate protection of concrete from extreme temperature and other environmental condition during
early stage of hardening and strength development which is non-compliant to SAES-Q001 sec.3.2
referring to ACI 301 para.1.7.4.1e. Hence, it is advise to construction team to investigate the extent of
the cracks and provide the proper corrective and preventive action.

On July 24, 2018, QC Inspector pro-actively notified the contractor team regarding the requirement of
additional side reinforcement of footing foundation having more than 600 mm thick, before the
installation of reinforcing steel bars for CF2 and CF3 foundation with 700 mm.thick in Engineering and
Maintenance building. In spite of advices. Construction completed the installation of top and bottom
rebars without providing side reinforcement. /this is an obvious violation to the requirements of SAES-
Q-005 sec.4.3.2g.

I 10-02399-0006 Yanbu Distribution Hub CMEI Package During Surveillance on 05-August-2018 at Pipe
Sleeper 76, It is Observed thatco nstruction team has started Installation of formwork for pipe sleeper at
SLP-L-74 & SLP-L-75,Chipping for construction joint between pipe sleeper foundation and Pipe sleepers.
Area to receive Concrete is not properly roughed and chipped (as shown in attached Picture) which is
nonconformance to ACI-301-11 Sec 5.3.2.6 where stated rough the surface in an acceptable manner
that expose the aggregate in uniformly

During routine site surveillance near pipe sleepers SLP-L-190 on 15 Aug.2018. It was observed that
construction team started backfilling and the used have gravel more than 50mm (see attached photo). It
is clear violation of SAES-A-114 sec.6.1.2.

During site survelliance on 09 september 2018 at pipe sleepers L-line minor honey combing found(see
attached pic) during form work stripping for pedestal's in SLP-L-76 due to improper use of vibrator. It is
violation as per SAES-Q-001 Sec. 7.23.8. Which state that placed concrete shall be consolidated by
mechanical vibrating equipment according to ACI301-10, sec. 5.3.2.5

10-02399-0006 Yanbu Distribution hub CMEI package during survillance on 09-sep-2018 at utility power
sub staion it is noticed that there are some rebar are protruding out from lean concrete and it cause the
reduction of concrete cover from most outer edge of epoxy coated rebar. It is clear violation of SAES-Q-
001 SEC.7.18.9 table 3 where mentioned miniumum concrete should not be less than 75mm from outer
most rebar

uring site visit at pipe sleeper foundation, Grade 'D' on sep.10,2018, It was noticed that the exposed
concrete corner of foundation# SLP-D-03, 010 & 12 was damaged which may be bumped by formwork
stripping a hard object. This is violation to the requirement of SAES-Q-001, SEC 3.2 (ACI 301-10- Sec
1.8.2), which state that concrete shall be protected from damage by mechanical disturbance, including
load induced stresses, shock and vibration. Construction team is herbey advised to repair the said
damaged structure to comply with the requirment.

I 10-02399-0006 Yanbu Distribution Hub CMEI PackageDuring surveillance at Receiving Sub-Station It

has been noticed that today dated 17-Sep-2018, 16mm (08 Ton) 20mm (12 Ton) Epoxy coated deformed
straight rebar 12m length arrived at site. Construction team used 20mm (4 Ton) rebar without receiving
inspection. This is clear violation of SATIP-Q-001-002 item # 2 clause 2.1 & 2.2 and SAES-Q-001 Section 7
clause 7.18.

10-02399-0006 Yanbu Distribution Hub CMEI Package During surveillance on 18-Sep-2018 at Fire water
Tank it has been observed that construction team has start repairing work of Fire water tank concrete
surface without any Grinding, chipping ,and V-Cut around deteriorated concrete. It is non-conformance
of Approved MS Repair of concrete Structure with Document Transmittal NO: SSEM-YDH-T-2030 SEC.
8.1 Where stated all bubbles/ cavity surface shall be chipped/brushed out and the surface thoroughly
clean.

I 10-02399-0006 Yanbu Distribution Hub CMEI Package During Surveillance on 25-September at Pipe
Sleeper 80 and 78, It is Observed that construction team has installed embedded plates with support of
wooden piece and with uncoated rebar (as shown on picture) that may prevent the concrete bond it is
violation of ACI 318-10 SEC 7.4.1 Where stated that at time concrete is placed, reinforcement shall be
free from mud, oil, or other nonmetallic coatings that decrease bond. Construction team is advised to
comply with ACI 301-10 SEC.6.2.1.6a use no corrodible, stainless steel, plastic, or plastic coated wire
reinforcement supports and spacers near exposed surface.
