PREVENTING URINARY TRACT

INFECTIONS RELATED TO URINARY


CATHETERIZATION
Purpose: To provide nurses employed at Bon Secours Saint Mary’s Hospital
operating room information necessary for Properly inserting a urinary catheter
ü Goal: To increase knowledge about the insertion procedure for urinary catheters,
and the indications which warrant its use.
ü Goal: To demonstrate the ability to aseptically place urinary-catheters following
evidence-based guidelines.
ü Goal: To overcome bad habits of seasoned nurses and resistance to change.
COGNITIVE/KNOWLEDGE OBJECTIVE

• Upon conclusion of the teaching session the learners will be able describe what
CAUTI means, to list the indications for urinary catheter use, to identify the
sequence of events and supplies needed for placing urinary catheter and list key
practices nurses can use to help prevent CAUTIs.
CONTENT TO MEET KNOWLEDGE OBJECTIVE
• What is catheter associated urinary tract infection (CAUTI)
• “A urinary tract infection (UTI) is an infection in the urinary system, which includes the bladder and the kidneys. If you have a urinary
catheter, bacteria or yeast can travel along the catheter and cause an infection in your bladder or kidney” (CDC, 2009)
• Indications for placing a urinary catheter (CDC, 2009)
• Acute urinary retention or bladder outlet obstruction
• To improve comfort for end-of-life care if needed
• Critically ill and need for accurate measurements of I&O (e.g., hourly monitoring)
• Selected surgical procedures (GU surgery/colorectal surgery)
• To assist in healing open sacral or perineal wound in the incontinent patient
• Need for intraoperative monitoring of urinary output during surgery
• How to prevent CAUTI
1. Use fewer catheters (CDC, 2009)
2. Timely catheter removal (CDC, 2009)
3. Proper care (insertion, maintenance, and post-removal care) (CDC, 2009)
• Steps and supplies needed to insert urinary catheter

PSYCHOMOTOR/SKILLS OBJECTIVE

• Upon conclusion of the teaching session the learners will be able to verbalize the sequence of
events when placing a urinary catheter, and demonstrate the aseptic technique of insertion either
in skills lab or during surgical procedure.

• Male Foley catheterization check list
• Female Foley catheterization check list

• Bard sure-step in-service video (20 minutes)


• Skills check to be performed in skills lab with simulation dummy or during surgical procedure and
checked off by coordinator or educator within 1 week of in-service (15 minutes)
AFFECTIVE/ATTITUDE OBJECTIVE

• Upon conclusion of the teaching session the learners will be able to share their fears related to the
introduction of the urinary catheter.

• List of reasons nurses may be resistant to learning and placing urinary catheters discussed (5
minutes)
• Group Discussion (10 minutes) with educator suggested topics
• What is differences exists in the process of inserting urinary catheters from what you previously
learned? How does this make you feel?
• Will you implement the information into your personal nursing practice? Why or why not?
• What barriers do you perceive in implementing these strategies?
• What is your anxiety level or lack thereof related to competently placing a urinary catheter using
aseptic technique?
