Greetings in the name of St. Thomas Aquinas!

We are Grade 12 Health Allied students from the University of Santo Tomas who are
currently undertaking one of the requirements in our subjects: Introduction to Health Care and
Work Immersion. One of our requirements in the said subjects is to conduct a recorded, semi-
structured interview with any of your medical personnel to familiarize ourselves with the
different roles of health care professionals from various health care settings.

In view of this, we humbly seek permission from your good office to allow us to hold an
interview with one of your medical personnel on September 18 (Wednesday) at around 4:00PM
or September 20 (Friday) at around 4:00PM as well. Please be assured that the data gathered will
be treated objectively and with utmost confidentiality. Your assent is the fruition of our
objectives. Attached is the list of guide questions to give the medical personnel a preview of
what is going to be asked.

Kindly let us know if there are other requirements that we need to comply with before

2. What are your roles and responsibilities?
3. State your personal experiences in the area (working environment, working with
colleagues)
4. State your professional experiences in the area (successes, difficulties, fulfilling
experiences, realizations)
5. What are the TOP 3 commonly encountered diseases/illnesses/disorders/health problems
in the area/field?
6. What do you think are the factors/causes that have contributed to the development of the
diseases?
7. Is there a possibility to prevent these top 3 health problems? If yes, what are these
preventive measures and how do you classify them according to levels of prevention?
