AFFIDAVIT

I, DANILO ENOC AMBAN, of legal age, Filipino, married and a

resident of Lot 18 Isaiah Street, Emily Homes, Barangay Cabantian, Davao
City, Davao del Sur, after having been duly sworn to in accordance with law,
do hereby depose and say:

That I am the Secretary of an Indigenous Peoples Organization

registered as PRU-IP FARMERS ASSOCIATION, INC. with principal
address Purok 4-A, Limbaan, New Corella, Davao del Norte;

That this affidavit was executed to attest the none of our organizers,
incorporators, directors or officers were agent of or related by affinity up to
the fourth civil degree of the official of the Department of Tourism (DOT)
of the Republic of the Philippines, or any agency authorized to process
and/or approve proposed MOA and release of funds;

This is in connection with the compliance of the checklist of

Documentary Requirements for Non-Government Organizations, Peoples’
Organizations and Private Organizations Special Project Review Committee
(SPRC);

That I am executing this affidavit to attest to the truth of the foregoing

facts and to inform the proper authorities concerned regarding the incident.
