How to Deactivate Clients

1) Review chart and make sure everything that needs to be in is in, and they haven’t come in at all
and a significant amount of time has passed despite outreach. (eg. 180 days.)

2) Write a Special Use Progress Note (you can copy paste this template if it’s easier) and route it to
myself and Abby

PSW completed discharge summary for the purposes of case deactivation.

DEACTIVATION SUMMARY

A) TREATMENT SUMMARY
Client was opened in the Edelman CORE Outpatient program on INSERT DATE HERE with
a diagnosis of INSERT DIAGNOSIS HERE and assigned to a case manager YOUR NAME
MSW Intern and a psychiatrist DR. SO AND SO (also can add therapist here eg. “and
provided individual therapy by so and so from x date to y date”) Client was provided
targeted case management and medication support services and whatever other
services they got, or you can write client did not return after intake to receive further
services if that is the case.

B) STATUS UPDATE ON TREATMENT PLAN OBJECTIVES


Unknown, as client fell out of treatment. (Unless you happen to know what
happened. Eg. The client’s treatment plan objective was to increase sleep from 2
hours to 7 hours a day; at the time client departed the clinic they were reporting a full
7 hours of sleep. The client’s treatment plan objective was to reduce SI from daily to
less than 1x a month, client reported no SI in the past 2 months upon case closure. )

C) REASON FOR TERMINATION


Over 180+ days inactivity (Or alternative justifiable reason—eg. Client moved to
Bakersfield, etc.)

D) REFERRALS GIVEN
Put here if you have any

E) FOLLOW UP PLANS
Client's case can be reactivated upon return to the clinic. Client should go through
Welcoming and Triage team for a new assessment.

F) OTHER PERTINENT INFORMATION


Anything else you think is important (eg. The client was advised of Edelman WMHC’s
clinic prescribing parameters around stimulants and anxiolytics, etc.)

3) Go to Primary Program of Service form and hit EDIT (not hit add) then go to inactivation and
put in the date, your name, and reason for inactivation.
4) Then go to Client Practitioner Assignment form and click “add” but instead remove everyone
from Edelman associated with the chart. Unfortunately, due to the way this is set up you will
have to open the add page multiple times to remove folks.
