This Contract of Lease is made and entered into by and between

MYRNA D. LATADE, of legal age, Filipino married, and a resident of
Cuartero St., Jaro, Iloilo City, hereinafter called the LESSOR;


___________________________________________ of legal age,

single/married and a resident of ________________________________________,
hereinafter referred to as LESSEE;


WHEREAS, the LESSOR is the owner of a certain parcel of land together

with one (1) storey house constructed thereon situated at Blk. 19 Lot 11, Phase I,
Bagong Nayo II, Antipolo City;

WHEREAS, the LESSOR is desirous of leasing the said house and the
LESSEE accepts to lease the same subject to the following terms and conditions:

That the term of this lease is for ONE (1) YEAR which will take effect on
____________________________ renewable at the option of both the LESSOR
and the LESSEE;

2. That the monthly rental of the said house is FIVE THOUSAND PESOS
(P5,000.00), Philippine Currency, payable every ________ day of each and every
month thereafter starting ____________________________;

3. The advance and deposit shall be retained and maintained by the

LESSOR and shall not be applied to any unpaid rentals, however, the advance can
be applied for the last month of the contract while the deposit can be refunded
only to the LESSEE upon surrender of the keys to the LESSOR and after all the
unpaid pending bills have been fully paid;

4. That the LESSEE agrees to pay the LESSOR one (one) month deposit
and one (1) month advance rental or a total of TEN THOUSAND PESOS

5. That the LESSEE agrees to pay the electric and water bills during their
stay in the said house;

6. That any minor repair introduced in the said premises shall be on the
account of the LESSEE and that any permanent repairs introduced thereon shall
be on the account of the LESSOR;
7. That any fixed improvements introduced by the LESSEE shall be with
the consent of LESSOR and that after the duration of the lease, the said
improvement shall remain with the LESSOR;

8. That the LESSOR shall be given free access to enter the premises at any
reasonable time of the day to conduct inspection of the property;

9. That failure of the LESSEE to complete the terms of this contract for
any reason whatsoever, shall forfeit his/her advance/deposit in favor of the

10. That after the duration of the lease, the LESSEE shall fix any minor
damage caused by fixtures introduced in the said premises.
