SELF-AFFIDAVIT

I, VILMA N. GACAYAN, Filipino, of legal age, married, and a resident of

Upper Torre, Inayawan, Cebu City, Cebu, after having been duly sworn to in
accordance with the law, do hereby depose and state:

1) That I am a Pag-Ibig Member and thus issued a Pag-Ibig No. 0701-

237035-01;

2) That I was an employee of Asian Cuisine Corporation (Majestic

Restaurant) located at Belvic Theater, Mango Avenue, from the year
1985 to 2000;

3) That I can no longer get a certification from Asian Cuisine Corporation,

because the said company had already ceased its operation;

4) That I am executing this affidavit to attest to the truth of the foregoing

and for whatever legal purpose that this affidavit may serve.

JOINT AFFIDAVIT

We, AGUSTIN T. FEROLINO, JR. and JAMES LABAJO, Filipinos, of legal ages,
married, and residents of Bulacao, Talisay City, Cebu, Philippines, after being duly
sworn to in accordance with law, hereby depose and state:

1) That we know personally the person of MARJEE ROSE R. SEVILLA for

having been her long time neighbour and friend;

2) That we know for a fact that per record at the University of Cebu-Main
Campos, her name was entered as MARJIE ROSE R. SEVILLA and her
year of birth was likewise entered thereto as 1993;

3) That we also know for a fact that per record in her NSO Certificate of
Live Birth, her name was correctly entered as MARJEE ROSE R. SEVILLA
and likewise her correct year of birth was entered also as 1992;

4) That we hereby affirmed and confirmed that the name MARJIE ROSE R.
SEVILLA per records in her University of Cebu-Main Campus and the
name MARJEE ROSE R. SEVILLA per record in her NSO is one of the same
person;

5) As such, we are executing this Joint Affidavit to attest the truth of the
foregoing facts and for whatever purpose that this affidavit may serve.
