II. Evaluation:
Evaluation serves as the process by which data are collected, analysed, and interpreted so as to inform personnel management decisions related to teaching and
learning. Specifically, the purposes of evaluation are:
1. To inform employees of the performance expectations in Schools.
2. To provide data to assist in the continuous improvement of professional performance.
3. To measure the identifiable elements of teacher performance.
4. To communicate the evaluator’s assessment to the teacher.
5. To insure an on-site observation of teacher performance.
6. To provide opportunity for supervisor-teacher interaction.
7. To critique teacher performance.
8. To plan strategies to enhance the teaching-learning process.
9. To provide an avenue of communication between the school head and teacher.
10. To promote the achievement of district and/or school program goals and objectives.
Samples of data gathering devices or rubrics for classroom observation pursuant to the Section 5 of DepEd Memorandum No. 196, s. 2012, MONITORING OF
GRADES 1 AND 7 CLASSES OF THE K TO 12 PROGRAM are as follows:
a) Form 1 – Teacher Observation Guide for Instructional Competence;
b) Form 2 – Survey Questionnaire for the School Head;
c) Form 3 – Focus Group Conversation (FGC) with Learners; and
