AFFIDAVIT OF CONSENT

We, RODERICK B. ARELLANO and FELOMINO B. ARELLANO, both of legal age, Filipino
citizens and residents of Brgy. 8, Sarrat, llocos Norte, Philippines, under oath states and
depose that:

1. We are the owners of a portion of parcel of land, containing an area of 429.50 Sq.m.
designated as Lot No. 13485, Cad-621-D, covered by Original Certificate of Title
No.P-43008;

2. The we acquired the said parcel from the heirs of the late Rizalino Damo on August
31, 2010;

3. The above-mentioned property is subject for application for locational Clearance

of Zoning Compliance for Chappel Building;

4. The best of our knowledge, the above-mentioned property is free from any lien or
encumbrances;

5. FREE GOSPEL CHURCH OF THE PHILIPPINES INCORPORATED herein duly

represented by ANGELITO I. DEUZ, of legal age, married, Filipino citizen and a
resident of Brgy. 18, San Nicolas, Ilocos Norte desire to construct a chappel building
on the above mentioned parcel of land;

6. We are giving full consent and hereby permit FREE GOSPEL CHURCH OF THE
PHILIPPINES INCORPORATED herein duly represented by ANGELITO I. DEUZ to
erect a chappel building at the said parcel of land;

7. We are executing this affidavit to attest to the veracity of the above-mentioned

facts thereby satisfying the requirements of the HLURB relative to the application
for locational Clearance/Certificate of Zoning Compliance of the above mentioned
parcel of land;

8. We are also executing this affidavit for all intents and purposes it may lawfully
serve.
