Welcome to the Governor’s Honors Program (GHP) 2019 state application. Students must
complete all components of the application before it will be considered for state level
competition. The first task of the student online application (Provide Contact Information:
Primary Parent/Legal Guardian) is designed to collect the correct name and email address of

the primary parent/legal guardian. This will generate an email that is sent directly to the primary
parent/legal guardian alerting him/her to complete the permission form. The Primary

Parent/Legal Guardian Permission form MUST be completed by the primary parent/legal
guardian of the nominated student before the student can complete any portion of the online
application.

The GHP state application will be available to state nominees for completion beginning

November 16, 2018 through January 4, 2019. For more information about the program, please

visit the GHP webpage. All GHP applications must be completed using the GHP online
application system through FluidReview. No paper applications submitted in lieu of the online
application will be accepted and scored.

Provide Contact Information: Primary Parent/Legal Guardian

The Primary Parent/Legal Guardian form is the first task of the student online application.
Nominated students must provide contact information from a parent/legal guardian to begin the
online application. The application will ONLY be available in its entirety once the primary
parent/legal guardian has completed the parent task.

All videos will be uploaded into the GHP online application through YouTube. ALL VIDEOS
NEED TO BE MARKED AS UNLISTED WHEN YOU ARE UPLOADING THEM INTO
YOUTUBE. Please click Guide- Nominee Video Submission (Browser) or Guide- Nominee
Video Submission (Phone) for specific directions on how to use YouTube to upload an
unlisted YouTube video.

1. Introduction: At the beginning, SIMPLY state ONLY your first name, instrument, and
number of years you have played. Then describe your interest in Music and what you hope to
gain from attending the program. (Please be brief: 15 seconds is typical length for the

introduction. Please do NOT include your school name, school district, or names of other
individuals.)

2. Performance: Before your performance, state the titles and composers of your selections. The
maximum combined performance length for the audition is six minutes. When recording your
performance, keep in mind that the production quality of the video is not as important as making
sure that the judges will be able to see and hear you. Please be sure that you and your instrument
