CERTIFICATION
I, MARICEL SORIANO, of legal age, Filipino, with postal and business address located at
INSERT PERMANENT ADDRESS, after having been sworn in accordance with law, do hereby depose
and state that:
1. I am a MEMBER-EMPLOYER of HILLSIDE HUMAN RESOURCE MULTI-PURPOSE
COOPERATIVE (“PHRMPC” hereinafter for brevity), operating as service provider with
Certificate of Registration numbered NCR-MCFO-XXXX-1013-170-LR valid until 1 December
2020 issued by Department of Labor and Employment (DOLE) and registered with the
Cooperative Development Authority of the Philippines with Certificate of Registration number
XXX-16007029 issued last 26 January 2010;
2. I hold the position of Area Supervisor whose duty is specifically supervise the MEMBER-
EMPLOYER assigned/deployed by PHRMPC with the principal-client, SM HYPERMART using
the business name of KINGMIN INCORPORATION (KING hereinafter for brevity), located at
Manuel L. Quezon Avenue, Dapitan City, 7101 Zamboanga del Norte;
3. As Area Supervisor, my duties and responsibility, among others, is to ensure Member-
Employees have undergo compliance prior to its assignment, as follows:
a. Screening, interviewing, selection and orientation;
b. Receives compensation, benefits and working conditions in accordance to labor standards
as provided by the applicable laws and regulations;
c. Comply with our policies, rules and regulations that any deviations/violations would follow
appropriate set of corrective disciplinary measures after he/she is given the opportunity to
be heard; and,
d. And, for this purposes, Paramount conducts trainings and instructions for member-
employees to be fit and qualify for the work assignments assigned provided under our
service contract agreement.
4. HHRMPC member-employees assigned to KING are performing their duties and
responsibilities under the control and supervision of Paramount through my instructions on how
he/she will be able to meet the desired results of his/her assignments upon our approval. These
include, though are not limited to, matters such as employees’ applications for leaves,
authorization of overtime work to be rendered, authorization to work on holidays, and work
schedules and/or shifts of Paramount employees. These matters are never left to the discretion
of our clients, including KING.
5. We do not allow anyone from the KING managers, officers, supervisors and/or principal’s
agents to supervise or control HHRMPC member-employees in any way, which they sought
approval and permission should there be. If KING has any requests or needs something from
Paramount’s member-employees, they must communicate or coordinate with either myself or
the Area Supervisor.
6. We are also in charge of overseeing disciplinary matters concerning our employees, including
those assigned to KING. Our member-employees are hired and screened directly by the
HHRMPC. It is the HHRMPC that conducts interviews and other hiring procedures of
applicants that HHRMPC does not allow ng KING conduct pre-screening before we deploy/hire
anyone.
7. That, in the event that its service agreement of HHRMPC with the KING expires or is
terminated, the HHRMPC member-employees affected will be assigned within reasonable time
period in no less than three (3) months for MEMBER-EMPLOYEES’ security of tenure with the
HHRMPC is secured and uninterrupted. Unless the member-employee decides to resign and/or
finds him/her violated any policy, rules and regulations, he/she is afforded the opportunity to
be heard and appropriate disciplinary measure is imposed, inclusive of dismissal. But, if re-
assignment is not possible within 3 months, the Paramount will pay the proper separation pay,
in accordance with D.O. 174, Series of 2017.
8. That, I am executing this affidavit for whatever legal purposes it may best serve.
