    This cover sheet must always be included when submitting documents by regular post or a courier service. The information on this
    cover sheet helps University Admissions match the document copies you submit with your electronic application.

    Uploading documents

    Please DO NOT print and upload your cover sheet when uploading documents. When you upload, you're logged into your account
    which automatically matches your documents to your admission application.

    Submitting documents directly from your university

    If your degree was awarded in Bangladesh, Cameroon, Canada, Eritrea, Ethiopia, Ghana, Nigeria, or the USA (also upper secondary
    degree for USA), you must have your transcripts sent directly from the University.

    DO NOT include a cover sheet when you request your documents from your school as they cannot be accepted with the cover sheet
    included.
