resident of Brgy. Tibag, Tarlac City, after having been duly sworn to

1. I am the petitioner in the above-entitled case;

2. I have caused the preparation of the foregoing Petition;

3. I have read the foregoing Petition and the facts stated

therein are true and correct of my personal knowledge
and/or based on authentic/official records;

4. I have not commenced any action or filed any claim

involving the same issues in any court, tribunal or quasi-
judicial agency and, to the best of my knowledge, no such
