AFFIDAVIT OF ADMISSION OF PATERNITY

I, MARK JASON H. FERRER, of legal age, married, and resident of
Tempory Dy St., Brgy. Dayangdang, Naga City, Camarines Sur, after having been
sworn to in accordance with law, do hereby depose and state that:

1. I am the biological father of the child, ANNE KEYSHIA M. FRONDOZ0,

who was born on September 3, 2010 at Naga City;

2. That at the time of birth of said child, I am not married to her mother,
Anna Liza F. Ferrer;

3. That I hereby acknowledge my paternity/filiation to the child, ANNE

KEYSHIA M. FRONDOZ0;

4. That I am executing this affidavit to attest the truth of the foregoing

facts and for whatever legal purpose it may serve.
