 1. INTRODUCTION

1.1.1: Certified Lean Six Sigma Green Belt (LSSGB) is the third level formal training, covered under
the Lean Six Sigma methodology. It provides a thorough understanding of the process improvement
initiatives, under the guidance of qualified and experienced Lean Six Sigma professionals.

1.1.2: This is a unique program that has been developed by ’Lean Healthcare Initiatives’ with
healthcare content, which no other Lean 6 Sigma Certification organisation is offering. It covers the
entire Body of Knowledge as prescribed by American Society of Quality (ASQ) leading to their globally
recognised Lean 6 Sigma Certifications as well as Council for Lean Sigma Certification.

2. WHAT IS LEAN 6 SIGMA

2.1 Lean 6 Sigma Green Belt certification course is designed to give you an over view of Lean and 6
Sigma and organizational goals and show you how to DMAIC (Define, Measure, Analyze, Improve
and Control) methodology , measure current performance to identify projects and root causes of issues ,
generate and implement solutions with real life applications on various process oriented problems and
issues.

2.2: Through this program, Green Belt qualified professionals acquire competence and skills to
successfully train their peer groups in attaining the Lean 6 Sigma Yellow Belt level to work as Team
Members and complete projects which relate to process improvements within a function or department.
Green Belts can also be team members of large organisation wide projects under the leadership of Black
and Master Black Belts.

5.1 This course is beneficial for Operations personnel, Quality Professionals, Physicians, Nurse
Managers, Technician In-charges, Pharmacy managers, Laboratory and Diagnostic Executives, Front
office managers, Dieticians, Housekeeping Managers, Administration and HR executives and all
Healthcare Process Owners with a minimum of 2 years work experience.

i) Ashok Kunapareddy- Chief Lean Coach, Trainer and Lean Transformation Consultant, Lean
Healthcare Initiatives

Ashok Kunapareddy is an accomplished Master Lean Transformational coach and a highly experienced
trainer with a commendable expertise in achieving process excellence through Lean Healthcare and Lean
6 Sigma methodologies. He has conducted several national and international training programs and
workshops in these areas. He has been a resource person for Asian Productivity Organisation (APO),
Tokyo, Japan.

ii) Pavithra Singiri –Principal Lean 6 Sigma Trainer and Project Consultant (Healthcare), Lean
Healthcare Initiatives

Pavithra Singiri is a highly qualified and accredited healthcare quality improvement professional. A
Black Belt in Lean 6 Sigma, she is a highly experienced Lean 6 Sigma Healthcare trainer and consultant
who has carried out a number of Lean 6 Sigma Green Belt Certification Programs as well as executed
several Lean 6 Sigma projects for reputed hospitals nationally and internationally.

10. CERTIFICATE EXAMINATION:

10.1: Each certification candidate is required to pass a written examination that consists of multiple-
choice questions measuring the comprehension of the Body of Knowledge as prescribed by American
Society of Quality (ASQ)

10.2: The 6 Sigma Green Belt Certification Examinations is a 2.0-hour duration examination consisting
of 50 Multiple Choice Questions, conducted in English. It is an open book examination conducted after
the course directly or online. A candidate would be issued a Lean 6 Sigma Green Belt Certificate on
obtaining a minimum of 70% in the examination.

11. COST OF THE PROGRAM::

11.1: Rs 7,000/- + 18% GST per candidate for a minimum batch size of 8 candidates

11.2: Rs.6,500/-+18% GST per candidate for a batch of 9-12 candidates

11.3: Rs. 6,000/+ 18% GST per candidate for a of batch of 13-20 candidates

12. OTHER TERMS AND CONDITIONS:

12.1: The organisation needs to provide travel and stay for two consultants, for outstation
programs other than Bangalore.

12.2: The organisation needs to provide the Training hall/Conference room, Multimedia
projector, screen, White board and the Flip chart for conducting the program.

12.3: Participants are expected to come with laptop to the program.

12.4: The organisation needs provide the Working Lunch/Refreshments /Coffee/Tea etc., during
the program for the participants.
