Purpose of the Inspection:
 To identify the strength and weakness of the health and safety management
system.
 To identify the uncontrolled hazards and strength and weakness of the risk
control systems placed in the company.
 To identify the compliance with the legal and statutory bodies
 To improve the health and safety performance of the organization.

Factory Details: 4 Shop bays, 1 machining workshop, Blasting and painting yard, Material
storage yard , Office buildings, New Erecting site.

Head office details:


1 main building and new under construction building.

Site Details: 1000 sq and build up area is 500 sq.


Details: The company have 30 sections of the work areas, I have inspected up to around
10 areas.
Employee Details:
 6000 workers,
 50 managers
 320 engineers
 150supervisors,
 2000Foremen
 15 safety officers are working in this project.

There is no presence of vulnerable workers in the site.

During the inspection time period several observations have been identified. The control
measures may directly or indirectly make a impact on the financial stability and schedule of project
completion and more affect the current production and attitude of all staff.

As Safety is the key of the organization I strongly believe and insist to practice better safety
ensuring the welfare of the employee .

Executive Summary:
In this report, some of the major hazards and controlled measures are identified. The following
list may cover the important areas where the recommendations are required
Main findings of the inspection:

 Observed in shops that persons are using empty paint drums as stands during welding
and cutting activities , which are highly dangerous as the paint drums are not thoroughly
cleaned which in turn may get ignite and result in fire causing great injury to persons and
damage of property .
 Scaffolding persons working at height found without using safety harness and also tools
not attached with lanyard to the harness, which may lead to severe injury or fatality.
 During confined space operations it was observed that external forced ventilation and
proper lighting were not provided, also no Hole watcher was available also LEL level was
not monitored continuously. This may lead to suffocation, accident or severe problem in
case of emergency.
 Welding and cutting activities using rope access at height were found without fire blanket
attached underneath and also no fire extinguisher kept nearby, which may cause fire due
to flames and result in injury or
 Blasting persons were found working without using proper PPE and without knowledge on
the hazards of the process and equipments.
 Waste Materials were not segregated separately to their respective types which while
disposing result in severe environmental pollution.
Recommendations:

 Provide all the necessary training to the employees with schedule time.

 Motivate the employees to follow the safe system of work and emergency procedures.

 Management has to provide proper training and information’s to the employees about
hazards and risks
 Make a audit plan on maintenance inspection of tools and equipments.
 Provide sufficient storage to store the materials in the workplace to avoid improper storage
and free from trip hazards
 Make color coding of all equipments mandatory and maintain separate procedure for the
same
 Motivate employees by incentives and bonus for safe work practices on monthly meetings
 Create separate bins/ Trash cans highlighting the type of waste materials to be disposed
on each bin.
 Arrange safety talk once a week for the and safety meeting once a month without without
fail.
 Rope acces shall be substituted in some areas for scaffolding

Cost:

 Blasting PPEs full set shall be newly purchased which will cost around 4,00,00 for a pair of
set.
 The work at height safety training expenses per person will be Rs.1,20,000 and the
required employees’ man-hours
 Purchasing of non spark tools aids to reduce the risk factor specially confined space
operation will cost Rs.1, 00,000.
 New gas monitors should be purchased for confined space woks where LEL should be
monitored inside continuously. it will cost around 2,00,000.
 New Fire extinguishers and fire alarms shall be purchased which will cost around 70,000
 CCTV cameras shall be purchased for vehicle monitoring 2,00,000
 Purchase / Fabrication of trash bins with appropriate color coding will result in cost of
50,000

The suggestions given based on the actual cost vs the benefits of the implementation as well as
the loss in case of accidents. If the company implements the above mentioned control measures
then it may result in a big safe practice for the company, employee and environment

Main findings of the inspection

Fire Hazard:
During inspection Fire Hazards were found. This was observed everywhere around the shop
floor. Empty Paint drums were used as stands and to hold jobs which during welding, cutting or
any hot work operations may cause fire and result in burns, injury, fatality and damage to
environment.
The management has to make procedure for the disposal of paint drums adhering to the legal
and judiciary requirements.

Recommendations & Cost:


 Proper segregation of empty drums shall be made in areas no near to fabrication.
 Proper disposal shall be made to authorised body adhering the environment policy
 Maintaining the areas free from paint drums by housekeeping and separate storage
area for the used drums may cost 1, 00,000.

Benefits of Implementation:
By this investment and implementation employers legal requirements make employee as safe
system of work at workplace. Injuries/Harms to people and damage to the property may reduce to
ALARP by this implementation process. It is the moral duty of the employer to protect the
employees from injuries and fatality.

Breach of Law:
As per the law of ILO Occupational health & Safety Convention C155 Article 16, Employer
has to provide Safe place of work and more specifically the obligations mentioned in ILO
Recommendation R10 are possibly breached.
Work at Height:
Persons working in Scaffolding were found without wearing safety harness, if using were found
without attaching lanyards to anchor point. Also the tools used by them were not attached by
lanyard to the harness. This may result in fall of persons from height or drop of tools or
equipment which may result in injury, even fatality of below persons.

Recommendations & Cost:


The training expenses will be Rs.20, 000 per day which may take around 3 days to provide for all
staff.
Purchase of approved full body harness with shock absorbers and extra lanyards will take
around 75,000.

Benefits of Implementation:
Implementation of this may result for a better safe environment for the people and environment,
The employer has the responsible to provide work at height training to maintain a safe working
environment.

Breach of Law:
As per the law “ILO - Safety & Health Convention C167 Article 18 the employer duty
do all that is reasonably practicable to prevent anyone falling from height, and carry out the risk
assessment .

Confined Space (Inhalation):


While person were working inside confined space forced external ventilation were not
found , Hole watcher was not found, continuous LEL was not monitored. This may result in
suffocation problem, communication break resulting in poor response in emergency situation.
Also will lead to death of persons in emergency.
All persons shall be undergone safety induction training prior to job.
Recommendations & Cost:
Only trained persons shall be allowed to perform job.
Separate hole watcher to be placed available all time during the operation.
LEL shall be recorded and maintained in scheduled time.
The cost for the new gas detectors and forced fans shall be around cost will be Rs. 2,00,000.
Breach of Law:

This is the possible breaches of law ILO - Safety & Health in construction Convention
C167 Article which needs the safe electrical equipments at workplace.

Fire Hazard:
It was found on major areas that during welding and cutting activities no fire extinguishers were
placed near, also fire blankets were not available. Hot works were carried in some areas near combustible
substances without segregation.
In case of fire it may be difficult to extinguish and may lead to easy of fire spread resulting in injury and
property damage.

Recommendations & Cost:


Maintain proper house keeping
Provide fire proof screens incase to carry out hot works near combustible substances
The purchase of Fire Extinguishers and fire blanket may cause around cost Rs 10,00,000.

Breach of Law:
As per the law of ILO Occupational health & Safety Convention C155 Article 16, Employer
has to provide Safe place of work and more specifically the obligations mentioned in ILO
Recommendation R10 are possibly breached.

Blasting PPE:

Persons carrying out blasting activity were found without wearing proper blasting
PPE. This will result serious injury, harm and even fatality to the blasting person. The company is
intended to follow all safe act to protect environment and people.
The company stands for the responsibility to provide enough training on blasting process,
Hazards involved and provide enough PPE.

Recommendations & Cost:


The cost for buying new blasting PPE will be around 2, 00,000 Rs.
Permit shall be issued only after ensuring the availability of all PPE and safety checks.
Breach of Law:
As per the law, ILO Occupational health & Safety Convention Article 4, employer is
responsible for reasonable safe of their own. it shall be to prevent accidents and injury to health
arising out , but the law is breached.

Improper segregation of waste materials:

During inspection it was found everywhere including offices that, all domestic , industrial materials
were placed in a same trash area. This is breach as improper disposal of industrial materials may
result in environmental pollution and harm to living begins.

The management must take immediate steps for segregation of materials and make ensure all
employees follow the same.

Recommendations & Cost:


New basket/ Trash bin shall be placed wih sign boards placed on the type of materials to be
disposed or may be color code, provided the same shall be explained in induction;
Sign boards and posters shall be placed.
Fabrication /Purchase of trash bins may lead to cost of about 1, 50,000..

Breach of Law:
As per the law of ILO Occupational health & Safety Convention Article 4 the waste material
should be properly segregrate it and should have be some proper materials to segregrate the
waste materials.

Conclusions:

The observations clearly show that the strength and weakness of an organizations risk
assessment. The management has invests and provide many materials to protect their
employees from hazards and risks. But still the management wants to focus on the employee’s
attitude towards the safety of environment and make them realize the consequences of absence
of control measures.
The recommendation includes,
 Arrangement for Health & safety related training,
 Regular monitoring & improve the level of supervision,
 Conducting tool box talks regularly
 Provision of adequate personal protective equipment’s
 Provision of adequate fire extinguisher’s for suitable works
 Maintain the good housekeeping in organization
 Provision of proper storage for materials used in organization

The recommendations and control measures are strictly followed, then employer will
receive the economical and moral benefits, such as increase in safety culture & moral among
workforce will result in improved production.
On the whole, implementation of control measures and remedial actions
reduces accident and health related diseases will result in improved productivity of the
organization.
The employee & the employer morale is the best source to achieve the safe system of work in the
organization’s.
