Dear Mr. & Mrs. Happy:

My name is Anissa Borgens and I am Rebecca’s US History teacher this year. Rebecca is a
bright student with so much potential. This quarter I’ve noticed a few issues that I would like to
work with you both to correct. My greatest desire to build a relationship with you both so we can
work together as a collective team to ensure that Rebecca receives the best education.

Lately, Rebecca has failed to turn in assignments and does not seem to want to participate in
class. Over the course of the last month, Rebecca has turned in a total of two of the five that have
been due. These are the assignments still missing:

Discussion based question (due 9/5)

Discussion based question (due 9/12)
WWI Research Analysis (due 9/26)

As end of the quarter approaches, I wanted to reach out to ensure Rebecca’s success moving
forward and make you aware of the issues and potential consequences.

I want to inform you that Rebecca still has the opportunity to turn in any assignments she has not
completed. Assignments that are turned in late will not receive full credit, however students are
able to turn in any late work by the end of the semester and receive half credit. This is a rule in
my class and will be honored in this situation as long as those assignments are turned in before
December 5th.

Once again, I want to emphasize that my goal is writing this letter to help build a team for
Rebecca that is here to support her academic success. I would love to meet to discuss this further
in person. My email is a.borgens@highschool.com. Please feel free to reach out with any
questions or concerns. I look forward to meeting with you both and working together.

Sincerely,

Anissa Borgens
High School
