The MST (Mentor School Teacher) will use this form to formally observe the trainee’s performance and to give
formative feedback based on the selected teaching competencies. The use of letter grades is optional.

Comments:
The student was able to adapt the lesson provided by the teacher to create her own teaching ideas. She was well
prepared and lead the lesson with confidence and has great management over the class.

Comments:
The student had prepared all the resources well before the lesson and set up the centers easily while the children were
engaged by the story online.
Managing Learning F D C B A
Comments:
She worked well with each group, attending to their individual needs and supporting learners that may have been
slightly distracted or forgot the all the steps in the activity. The children behave well during her lessons as she has
already established a close relationship with them all and implements classroom routines that they are familiar with.

Comments:
The learners enjoyed all the activities and the hands-on interaction. Additional focus can be given to the Math skills
such as counting, detailed number/color/shape recognition, and use of positional vocabulary to allow the children to
gain fully from all the math concepts integrated into this activity.

Comments:
It is suggested that a checklist or anecdotal notes be used during an activity like this to assess the learners’ skills and
knowledge of the content.

Comments:
The student is encouraged to share more of her thoughts (through writing or discussion time) after the lesson to help
her develop her reflection on her teaching skills at a higher level that can enhance her teaching fully.

Action Plan (next steps):


-The activities can be made more challenging with some extensions and more focus can be given on the
mathematical skills.
-These crafts would be lovely for an art lesson that integrates the math skills.
-Assessment tools such as checklists and anecdotal notes can assist the teacher to receive better feedback
with the learning that occurred.
